You might try Rockler or woodcraft. Maybe they have the actual slide and you could just replace the whole thing.

There's a "new version" of these jigs on amazon, but no indication as to what the "new" features are. Any ideas?
AND if you’re doing face frames, take the lower doors and hinges off so they don’t get in the way, and use a chisel or putty knife to get rid of any ancient glue that is hanging around, and pre-drill your slide mounting holes…

How do I install the bottom drawer with the jig?
The bottom drawer slides cannot be installed with the jig. You need to use wood scraps as spacers to lift the drawer slide off the bottom.
Hi. I'm thinking of buying this jig, but can't figure out how it works on the bottom slides. Is it in the instruction booklet? Do you know how? Thanks a bunch.
If you mean drawer slides that mount on the bottom of the drawers I don't believe this jig will work with those.

when i clamped it on it was not level on the inside and i checked everythings around it to make sure it was square and level

That’s probably missing because why use old technology drawer slides when updated full extension easy close slides are far superior?
Some projects you don’t have to use more expensive slides and the old ones fit the bill.
used these all weekend, total waste of my time. A piece of wood and a clamp, or another pair of hands, that's all. Only thing these things are good for are cabinets in new or like new condition that were made right to begin with. My old ass cabinets made by a blind troll are easier done by the eyemometer. Also, the drawer install is only good for drawers without overhangs.
who even uses this type of drawer slide nowadays ? the ball bearing ones are far superior in everyday and still reasonably priced.
I see you've got two those $40 self adjusting clamps that don't really work all the time, I really hate these things, I really do. I wouldn't buy another, NEVER!
I paid $20 each for mine and I have had no issues with them. I appreciate you sharing your experience with the clamps.

It is strange and frustrating that kreg jig doesn’t address the use of these slides!
I guess whoever wrote the owners manual didn't think to include a section on epoxy drawer slides but the designer of the actual jig made a special shaped channel for them.
