Causes of delayed seroconversion or negative HIV test post 6months of exposure-Dr Ramakrishna Prasad

- The fear or the concern that one might be infected, going through an exposure of any kind, maybe it is a needle stick injury, maybe it is a sexual assault, maybe it is unprotected sex, whether it is for men or women, it is for a deeply traumatising experience. Because there are a whole range of emotions that come with it, there is guilt, shame, stigma, there is a sense of why did this happen to me etc, it is a complicated set of emotions and people are scared to share this with too many people. The facts are that after an exposure by about day 15, it could be a day earlier, if you do an RNA-PCR test and if it comes negative, the chances that you are infected are extremely low. By the end of 6 weeks, if a 4th generation antigen antibody is negative you can almost be sure the HIV infection has not occurred and by all protocols if there is a negative test at the end of 3 months, you are cleared of HV infection unless new exposure has happened. Some of the older guidelines have 6 month test after which again one would be cleared, this phenomenon of delayed seroconversion is not something that is expected or known in the literature if you look at the internet, we look at several sources or videos talking about people who are infected and test came back as positive after several years usually there is missing data or missing information in these cases. Rather than focus on the delayed seroconversion, I would focus on why someone continues to believe or continues to live with this anxiety or fear that they might be infected. It is just that these tests are not showing up to be positive. Infact there is a name for this called as HIV phobia. If you are experiencing any of these symptoms, my advise would be please go and meet a team which has both an HIV expert and a psychologist in an integrated manner and sometimes it requires a customised programme developed for you where a number of skills are taught along with the right level of medical supervision and if counselling lasts the right therapy, with the expertise of an HIV physician is combined many times there is an excellent outcome. So I encourage you to do that.

@@usmannaseer9292 don’t panic and try and take your mind off the worrying. What’s done is done. The only way you can know for sure and get peace of mind is by going to the clinic and getting tested. Unfortunately, though 80% of newly exposed patients will experience symptoms (seroconversion). So being healthy and showing no symptoms is not an a definite science. Just get yourself to the nearest place and get tested. Wishing you the best.

The hiv phobia brought-me to surf several videos related to a risky hiv exposure after I accidentally went through a NSI of an hiv patient who was on on treatment one year ago. This is really a Deep traumatising experience for me as well as for my family. I went through PEP for 28days, had CMIA Ag/Ab test for Hiv after 38 hours of exposure, at 3months and finally at 11months. Luckily all came back non reactive! But still I was so much in stress like may be I have got infection so went to see a physician for this and they said the same like Dr. Ramkrishan prasad mentioned in this video. Now Iam relax by trusting my docs and majorly on my god. May no one catches this deadly virus.

Bcoz he had medical checkup before going on ship ( he works in merchant navy) dt time all reports were normal.
Nw he followed same procedure for next joining but report came dt he is HIV I +ve. Reactive test 2.12 (4th generation)

Yes safe
As of 2018, a 4th Generation HIV 1 & 2 duo/combo Anitibidy test (has a 95% accuracy in 30 days after possible exposure and 99.7% accurate after 90 days) along with a HIV 1 RNA QL TMA (99.7% accuracy after 30 days). Both test will be a blood draw taken to a laboratory for processing (not a rapid test).
The 4th generation HIV 1 & 2 Antibody test is far better than the previous HIV 3rd generation test, which required retesting at 90 days. The 4th generation duo HIV 1 & 2 Antibody test along with an HIV 1 RNA QL TMA (early detection test) will be given to be conclusive that you are HIV negative.
The HIV 1 RNA looks for the actual HIV DNA in your system and has pretty much replaced the Western Blot.
Both HIV vaccines SAV001H & HVTN 702 have both advanced to Phase III trails and look very promising! I wonder if those vaccines can be combined?

i had exposure 82 days back (torn condom during relation with unkown person)
i did HIV DNA PCR at 27th and 57th day and both are non detected.
i did HIV 3rd g test at 60th and 70th day and both negative
i did lastly 4th G test ( AG/AB , HIV 1 & 2) at day 80th and negative (less than 0.9).
are those conclusive cause i feel tired all time and continous myalgia and bone pain.

Ive been with someone with unknown hiv stat. We did mutual oral sex and after few weeks i experienced those symptoms but its a bearable symptoms. I force myself to get tested 5 weeks after its negative..but since its just 3rd generation test..I' ll need to go back again next week..quite stress and depressed now..

Me too. I tested for Hiv anti ag/ab combo test, hepatitis C and syphilis after 180 days, but the symphtoms continues. I received pep after 27 hours after exposure

@@monishamoni8920 you don’t have it be easy on yourself. I have seen some 20% people with no symptoms but had hiv. If you have tested twice. I don’t see any reason to be concerned. You are absolutely okay

How can I test negative Elisa and Western Blot after testing positive in 1986 and receiving AZT etc treatment for 30 years???????
2 possibilities.. you were never infected and NAT testing will be able to confirm that. Or you have a severely depressed immune system which has stopped making HIV antibodies.

Have you done a complete blood count? You will get a wbc White blood count with this & you can ask for a specific test just for your cd4 count which is a part of your wbc. Remember that stress will either lower your wbc or elevate it & you’ll get sick with both.

I took a 4th gen 1 &2 combo finger prick blood test 6 weeks after possible exposure and then an insti hiv finger prick blood test at 10 weeks(71 days) both negative. What are the chances this will be positive at 3 months / 90 days. I have so much anxiety.
Ahmed Jr I had sore throat and muscle ache couple weeks after and now I have persistent dry throat which scares me. Counselors seem to think I’m ok that the test would have shown something by now but 3 mths is the supposed mark to be sure. I am in terror.

Brown browny Jr there are different rapid hiv test out there. I took one that was 30 min and the “insti” hiv test is only a minute. Google it. Anyhow I have tested 4 times within the window and now worried about the 3 mth mark test..praying I’m ok.

The fact you should worry can be that even if the 4th g test goes negative after x mounths, you can have simptomes ovious like loosing weight, pain in join mussle or rash....but what if it s about a hiv rare type like N or P group? The test don t reach them...or what if you wrealy have a leasy immune antibody system? B lymfocites getting lower?!...it s yeassy to ressume to the fact that you are safe of hiv, even if you are maybe not...

Because Oraquick is highly sensitive to make sure it gets it right so it gave you a false positive that is why it says if you get a positive after the window period go to a lab and get more testing to confirm

I have query , I had unprotected sex with a girl with unknown hiv status in July 2017 and after 9 months I started to fall sick like sire throat and flue so I took hiv test in next month June 2018 normal antibody test twice from government hospital (naco)and it was negative so am I free from hiv bcoz between this gap of 11 months before test I was not involved in any sexual or other activities!

1) I have done HIV 1 DNA PCR qualitative on 78,94,110 days result is NOT DETECTED
2)I have done RNA PCR qualitative method taqmon on 110 days result is NOT
DETECTABLE
3) I have done ECLIA 4th generation P24 & antibody on 78,88,94,105,120,126,159,164,240 and result is non reactive
4) Also one rapid test on 32, 64 days result is negative
5) Hiv 2 Rt qualitative at 110 days is also not detected
6)P24 antigen detectable test at 110 days is also negative
7) RNA PCR quantitative at 126 and 164 days

U should be good bro, the cdc states 45 days with that test is conclusive if its lab test done through your vein. Experts believe that the test is conclusive post 28 days. So honestly i feel like your going to be fine but id just take the test so u can have that peace of mind. After this try to get on prep, Pray to God and learn from this lesson.
