There is a conspiracy theory that Muriel and Eustace died in the first episode, but Courage didn't want to let go or was so overcome with grief he is in denial and still believes they are alive and moves them around like puppets.
There is another theory that after Muriel and Eustace died Courage died from starvation and they are in the middle of nowhere because they are trapped in limbo and don't know they are actually dead.

I don’t think it’s happening through Courage’s eyes it’s just real like Gravity Falls. Dipper and Mabel aren’t imagining it until Grunkle Stan didn’t believe Dipper until he saw the zombies attack the Mystery Shack. Muriel and Eustace aren’t really scared because let’s not forget that they’re elderly. They’re too old to be scared. Except they are shown to get scared from time to time and the same with Courage. That should debunk the Courage theory.
