Cholesterol Update: LDL and Lp (a)
Вставка

- Dr. Steven Nissen, Chief Academic Officer, Sydell and Arnold Miller Family Heart & Vascular Institute talks with Dr. Leslie Cho, Section Head Preventive Cardiology & Rehabilitation about LDL cholesterol and statin intolerance. They discuss the full range of options available and what is in the future for LDL management. Then, Dr. Nissen describes the latest Lipoprotein a (also called Lp (a)) - what it is, why it’s important and a new research trial addressing this new frontier in lipid management.
Why so much focus on LDL especially just the total? Aren’t we past only looking at LDL reduction....love to know what your diet and exercise recommendations are...
thanks for talking about Lp(a), Dr. Ford Brewer here on youtube has done a number of videos on this subject. Apparently, most docs don't even know about or test for it..........
From this conversation it is sad the public still points to HDL when engaging in lifestyle intervention. Research shows it does not play a causative role in atherosclerosis and should not be a target for pharmacological treatment.
Nobody targets HDL. They worry about LDL and LP(a), which are proven to be linked to CVD
Yes, but it plays a protective role.
Yeah that's true but hdl does play a role. It just that in patients with CVD the main concern is reducing the ldl and thus hdl is put on a back burner. But remember no matter how high your HDL is you never want your LDL to be over 100.
Thank yo for recognizing the intolerance problem.
10:50 Lp(a)
Thanks🙂
Thanks!!!
I loved this video. Very enjoyable watching two brilliant people talk about this. However What exactly is statin intolerance? Is it muscle pain? Which muscle? Is it GI issues? Is it fatigue and tiredness? I wish they would be more specific so you can identify if it truly is your statin or some other cause that is responsible if you have some symptoms.
This is not a cholesterol update. It's a drug update!
So I have high LDL and I have higher than normal range Lipoprotein (a) but my CRP is extremely low? So, what gives?
👌🏻👍🏻👏🏻 valuable info!! Thanks!!!
How is it possible that Lipoprotein (a) is bad when there are articles on Pubmed that say Centenarians have elevated levels of Lp(a)
that could be in the genetics for a good reason as to be protective for the body? Please explain.
@Серёга Одинсон Are you sure about that? "Remarkably, one-quarter of the centenarians had high Lp(a) serum levels even though they never suffered from atherosclerosis-related diseases." 1998 G. Baggio.
I was put on a low dose of rosuvastatin and couldn't tolerate it, joint pain and muscle weakness. Now on Livalo low dose about 7.5 mg / week. tolerate it well. Also baby aspirin. Looking into Niacin as statins don't work on Lp little A of which mine is high.
Linus Pauling and Dr. Mathius Rath showed decades ago about what Lp(a) role is cardiovascular health. Learn about what these 2 men discovered and how to reverse CVD. Good luck on your journey.
pcsk9..... what this is, wasn't explained enough in this episode. thank you for the info.
Maybe low levels of Lp(a) are not that good?
The Lp(a) levels were inversely correlated with the CIMT in this population, suggesting that subjects with a low Lp(a) level may have a predisposition to carotid atherosclerosis. 2012 article: CIMT in asymptomatic subjects with low Lipoprotein(a) levels.
Where is the indication for pravastatin for statin intolerance ?
However bempodoic acid has clinically significant (0.5%) incidence rate of achilles tendon and rotator cuff rupture-not good especially for people who would typically be taking up an exercise programme at start of treatment.
How to lower lp(a) risk ? apheresis ?
Thanks for pointing time when Lpa dicussion begin
What diet do they do?
@MS in other words, you mean we should do what drug companies say ...
@MS plant base diet is good but carbs are the issue. Medical stream divided into two parts plant base and keto. So this is a bit confusing topic.
Are there any trails being run for lipoprotein(a) I live in anderson south carolina and have lipoprotein(a) with readings about 195, I can't get any help in lowering my cholesterol?
My lipoprotein a
Why animals don’t get heart attack but humans do???
they do if you feed them refined carbs, there are researches showing dogs developing atherosclerosis after being diabetic.
Most animals with the exception of primates, fruit bats and guinnea pigs manufacture Vitamin C in their livers. The afore mentioned animals have shown in clinical trials that if fed a western diet do develop CVD.
@@realimagevanity2289 So avoid heart attack by eating well with no sugar and carbs? no diabetes, no inflammation. Sounds like a winner.
@@Notme-tq4xs not really "Inuits" who never who ate less tha 10g of carbs. And primarily ate fish were also observed to have atherosclerosis. Also lp(a) is unaffected by diet in 90% of people. So in the end it all comes down to genetics. If you have genetically high lp(a) then no diet or exercise can help only medication can help
LDL is the trojan horse and these two have fallen head over heals... LP (a) is a problem but the mechanism is up for grabs... How about just a little fasting... oh wait! there's no money in that....
How can you participate in the trial?
Big Pharma research grants can make one very happy🤫
Hi Doc!
Is the ASO out now for Lp (a)?
What is it? Pills?
I'm thinking about it also
How do I sign up for the trial?
Hello, Has this been updated?
Great Information. I am on a Statin- 20 mgs Crestor and also taking Zetia. On only the Crestor (generic brand) my cholesterol went in Half and my LDL went in half. I go for more bloodwork in December. I have No Side effects, at least not yet. I have been on since April. Does that mean I probably am Fine with Statins? Also, do we know what makes it OK for some people and others can't tolerate? (Side note- I have an LPa of 373, I did not have a heart attack, but I do have 6 stents put in this year) I am also taking a lot of supplements. Like 20 a day.
When I took 20 mg Crestor my Lp(a) did go up, it's wise to find a lab and get your blood tested regular and play with your statins intake to see how you can take the best doses for your Lp(a) In my case Niacin helps to reduce my Lp(a) with 25%. You already use the best statin to fight Lp(a) others will only raise your Lp(a)
@user-cl7jx1xb2l I take 500mg twice a day and fast now for at least 12 hours a day. Yet my Lp(a) stays around 40mg, I seem not to be able to lower it more.
@user-cl7jx1xb2l I take 500mg twice a day. I fast for at least 12 hours a day. Still I can't get my Lp(a) lower than 40mg.
How old are you?
@@vrb340 I am 68 and busy with statins since my first heart attack on 46
Not ONE mention of lifestyle or nutrition factors. They should lose their medical licenses.
Drug companies want to sell drugs. You got it now ?
Where can I learn more about this LP(a) trial?
I just had my second heart attack at 43. The first was at 26. And this nasty thing is the reason.
Thank you for the video!
That's what I want to know too. Iv'e got my first heart attack at 46, second on 53 and third on 63. If it goes on like this my heart will start making noise with all this metal inside. Back to angina already I really need to know how to deal with this Lp(a) It's also no fun to finally find out by myself (no help fro any cardiologist) about this stuff, thanks to the people who are putting this on UA-cam..
What was your lp(a) level?
@@stuwhite2337 It's not even that high but I never had high LDL, triglycerides but my HDL is never high as well. I measured my Lp(a) in Thailand and it was 50 mg/dl
@@johntrampe thanks. My cardiologist told me my LP(a) was high at 37 mgdl. That was after he put me on Atorvastatin which raised LP(a). Best of luck.
What was ur LPa levels
does anyone in this country do lipid apheresis?
Statins are poison
they make money
Check your Apo b and Lp (a) 20 dollar test! Potential live saver!
What do you do when it's high then ?
@@Notme-tq4xs Seek a good cardio doctor and seek good medical advice. Maybe fix your diet
Wow. The comments are just chock full of people who've consumed so much misinformation about statins and cholesterol.
Dr in 2014 I got know first time that my LPa is 185 I am taking Rozavell 10 and also was doing workout and diet too it decreases to 110 ,but in Dec Dr told me to to decrease to Rozavel 5 mg as I had muscle pain, after 3 months when I got my test done my LPa had increased to 148 then Dr started again Rozavell 10 mg also one table added that's Bamdac 180mg Dr said it will help in lowering my LPa Faster ,but now after 3 months when I got my test done my LPA now has reached to 185 so I am really scared, as one of Dr had told me with high LPa people have lived even for 10 years & in year I too will complete 10years with high LPa pls and now med is also not working on me pls suggest what should I do to control my LPa & has anyone survive normally even after completing 10 years and above with high Lpa.Dr pls reply I am from India & because of my spine issue not able to do workout yes I am walking 4 km 5 times weekly.
@Serhio Lipo(a)
How much now ?
Animal Meat lowers LDL faster
First thing they mention are statins! “let’s treat this number...” thank you modern western pharma medicine🤨
Why wouldn't they? Did you expect to hear about a whole food plant based diet coming here?
And... AND... It a NEEEEEEW, statin. 🤑
