Titan shifters can control their healing abilities, at least with training. Remember when Reiner was bitten by a titan back in season 2? He kept his arm from healing until after he revealed himself and Bertholdt as the Armored and Colossal Titans in Warrior. Eren is doing the same thing here with his injuries. Which is fitting since you can hear the opening bit of the soundtrack that played during Reiner's breakdown in Warrior.
It’s really rare for people to not have figured out the reveal until this point. I myself recognized him as soon as I saw him in the 2nd episode but ig it might've been cuz i was also suspicious from seeing Jean in marley in the first episode near the credits screen. But majority of the people aren't looking for that detail and half of them figure out at the end of the third episode when he says his iconic line "keep moving forward" with a closeup on his face and the dramatic transition into the credits screen. Even then so many people don't figure it out
same concept for Annie in season one. From the manga I didn't know it was Annie until the confrontation with Armin and it was an amazing shock. But unfortunately with the anime's hair and eye coloring it was pretty obvious for a lot of people go guess.

The dialog that Eren used in the episode before when he was speaking to Falco in the hospital is one of my favorite dialogs in the show, I figured out it was him in that episode, which made the scene with his grandpa more epic with Eren triggering him

I think you not recognizing eren on episode 2 just made this reaction even more amazing. You literally felt the same surprise Reiner did. I personally liked it a lot more this way
