Easiest place is in the hero sandbox, open the item shop and click "Browse Builds" then click "Create New Build" in the top left side of the screen, from there you should be able to create new categories and add items to them.
So when you buy a item that has a component, do you get the effects of both items or only the parent item? Because some items that are/have components have different active and passive boosts.
Once you upgrade the item you only gain the benefits of the new items actives/passives. Most items are just a upgraded version, but like you said, not all of them.

What's an Item you can't live without on every hero? My go-to is Active Reload right now!
Extra Stamina is just so good on almost every character right now, extra movement feels so valuable.
