    Drug resistant TB strains are rampant in the DPRK (North Korea). This seems to me to result from limited resources and the best of intentions. For example, suppose a very sick patient is appears at the hospital. The complete regimen of treatment may be, for example, 4 weeks of Drug A. But 2 weeks of Drug A is enough to save the patient's life. Meanwhile, a second patient appears who is also very sick. 2 Weeks of Drug A is enough to save his life as well. Two lives have been saved. But neither patient has received the full regimen of treatment, with the result that we have just strengthened the pathogen in both patients. Caveat, I'm not a doctor, but I've seen this happen specifically with respect to TB treatment in that country. The real culprit is the lack of resources, exacerbated by the politics of "sanctions" which always result in the poor suffering more, in my opinion.
