Imo, tainted judas and keeper have the best ones but forgotten has some real cool ones
The guy in charge of Tainted Judas's sprite variations deserves a raise.
Sag did an amazing job bringing these up
tainted judas's ones are nor realy special tho, i made a list of the special visuals with blue baby, the forgotten and possibly keeper too.
here is the list:
blue baby has a diffrence apearence with smb super fan, and the forgotten does as well, and forgotten has a difrent apearence for ball of tar too.
steven has difrent apearences with blue baby, the forgotten and tainted judas, and with lost, azazel, and esau it only changes the skin color.
leprosy, tough love and jupiter have a difrent apearence with blue baby.
the mulligan has a slightly difrent apearence on blue baby and the forgotten (it changes their teeth)
also the forgotten has a difrent apearence with brimstone + a completely difrent attack.
the forgotten also has a difrent apearence with monsto's lung, mutant spider, mars, capricorn, polyphemus, ipecac, pyromaniac, the common cold and the inner's eye backeye.
also soy milk and chocy milk have a slightly difrent apearence on blue baby, forgotten and i assume keeper to fit their mouth.
the inner eye turns into an x on blue baby, and into a hole on the forgotten.
serpents kiss has a difrent apearence on the forgotten, and might have on blue baby too.
also tainted blue baby "technicly" has the same difrent apearences as the normal version, what the devs did is they took the normal blue baby's skin and sticked the mouth and fat body on top of it, so those dont change.
everything i said about the forgotten could apply to keeper too.
tained judas is pretty cool tho

but yet birthright doesn't work on most characters and golden trinkets are useless most of the time 😂

Edmund wants it to be spelled Time Fuck as seen in the Basement Collection title for it glitching, but newgrounds and steam would not allow it
Vade Retro is just straight up a melting from nuclear throne reference , you can even use it one last time after dying just like melting
dont forget that the big explosions from killing boss enemies are 3 smaller explosions in a triangle shape. just like meltings throne butt effect.
I have to remind you that "birthright" is the same as "throne butt"?

Kinda late, but that's most likely because, Killburn (one of the devs) said that his favourite Nuclear Throne character is Melting.

Tbh cursed eye has saved my keeper runs and kill delirium and hush with it
maybe they had black candle already
A reminder that if you have Black Candle, Cursed Eye just becomes free machine gun tears with no teleportation risk
Fun detail/synergy for the next video: Rotten Tomato causes Dogma to attack its own TV or the flies of the ultra pestilence in the beast fight attack him and he shoots himself with his own shots
"obese horseman" is how I will refer to Ultra pestilence from now on.
That's the point of the item though
The best part is that if Dogma's TV dies while under the effect of Rotten Tomato it gets the effect permanently. So no more worrying about dodging the feather wheel attack if the feathers always spawn from the middle of the screen.

You rarely actually get to see many of those Easter eggs in game, since because of all the items you get you look like a monstrosity most the time and are not able to understand anything that was once your character
Another fun synergy is having the safety scissors during the beast fight against war

Yay, another one of these! I love these little details. Allows us all to appreciate all the care the devs put into all these synergies between characters and items, and between items and other items.
Maybe you could one day do one on rare rooms, too? I heard from an interview with the Repentance devs that they put a lot of those super-rare rooms in. Might be cool to see... But I don't know if there's a reliable way to make them appear?

Most of appearance-changing items work in a very interesting way with dark/tainted Judas. Once I was doing a challenge where you have blindfolded (don’t really remember which of them) and I turned into a dark Judas and got the conjoined transformation at the same time. So my character looked like something from the depths
combining The Intruder and Transcendence gives a weird effect as well, if you get hit and the intruder pops out, you're lef with no head and no body, so isaac will just be invisible for the remainder of the room
