    Advanced Care Planning for Healthcare
    What healthcare do you want if you’re critically ill? How will your doctor and family know your wishes if you’re unable to communicate at the time you require care?
    Everyday, people are hospitalized and are unable to tell their family or medical team how much or what kind of healthcare they want to receive, or when they want care to be discontinued. They haven’t expressed their wishes and priorities, leaving loved ones and doctors to make critical decisions in an information void.
    Advanced Care Planning is a process of considering the possibilities and providing direction for healthcare decisions if or when we’re unable to speak for ourselves at the time care is needed. In this 2-hour online workshop, we will:
    Identify our priorities and boundaries for medical care. How much care is the right amount for you personally?
    Learn ways to communicate what’s most important to us, should we be unable to speak for ourselves.
    Explore ways to talk to the people in our lives who will represent our wishes if needed.
    Examine the challenges a representative might face and how advanced care planning can ensure your wishes and personal values are honored.
