In most instances, the Midline is run as a double option. When run from a 2 back set (I formation normally), the tailback is often used to isolate on an LB, making the QB keep more like a QB Iso. Pistol and Spread teams often pull a backside guard as a lead blocker for the QB to accomplish the same thing, or to kick out the man responsible for the QB.
