The only equipment that does anything during the Sans fight are items that increase your invincibility frames, that just slows down the KR effect

it would technically miss because if you disable the dodging on sans and attacked him, he would take damage but still be alive, thats because he doesnt have a death anim.

The “Did you ever get something to eat” line during neutral ends are for completing UnderTale without using any items, not for having no armour equipped. (It isn't even possible to have no armour equipped, anyway, so why would he have challenge dialogue for it?)
No, sans’ dialogue “you really like to challenge yourself, huh...? well... good job. just promise not to brag about it, okay?" is different from the no item run. It happens when you meet three conditions.
1. No using items
2. No equipping better armor
3. No equipping better weapons
However, since the code just checks to see if you’re still wearing the bandaid, it sees that you aren’t, and the dialogue doesn’t play.

I meant that the Sans' “you really like to challenge yourself” quote has nothing to do with the quote for not using any items in the run. They are two different quotes for two different challenges. I *_am_* correcting the video.

I did this before that video, I thought that Papyrus was reffering to Frisk clothing, but after knowing he was calling it bepis... I REALIZED FRISK WAS NAKED

On my latest pacifist playthrough I did this, by never taking off the bandage. Game is way harder like this lol
Tip if anyone’s doing geno: best items for sans are cloudy glasses and burnt pan. DEF doesn’t matter for sans but INV does(a little) and the burnt pan gives 4 extra HP for each healing item, also get the instant noodles, they can literally heal you to full even at LV 19
Edit: they heal 4 hp in normal battles, 15 outside of battles and 90 in the asgore, toriel, asriel, sans and undyne(geno) fights

Maybe you or someone has done this b4, but I haven't seen it. Will sans still talk about resets if you reach him on geno without ever having reset, SAVEd, died, reloaded, any of that? So, theoretically, like someone reaching sans on their first-ever run on a new console without dying anywhere along the way. I don't know if hacking to get there would screw up the results, tho (I mean, since sans is the one who calls you a dirty hacker).

Actually, there are 93 endings even without this.
Betrayed Undyne with date with challenge (lv1, no armors, no consumables except the pie, you must play pacifist until after you date undyne, then kill a monster while staying at LV 1. Problem is, all monsters in Hotland and Core give enough EXP to level up above that. EXCEPT SO SORRY.

I believe sans fight ignores def to make the fight the exact same whether or not you are prepared for him and being lvl 19 its unavoidable anyway
