The abrupt transitions are a bit like a superhero movie. Its like he's got these powers and has no control of them and there is always the consequences of the mischief and it just follows him, then by the end of the vid he figures it out and starts to intentionally use it. Like he's accepted the mischief and chaos. Embracing it instead of trying to hide from it. *Using* it. I also think the apple is a metaphor for making better decisions for his own health, especially after spitting lines about his troubles with doing exactly that in other tracks. You mentioned reality, and this is my first vid of yours I've seen so maybe you know this already, but I'm almost certain that his other vids, in particular the ones where he's carting around balloons (and the cart) are all in his mind, they're just other rooms in The Mansion.
