Example there was a influencer who posted lots of pictures and a keen eyed fan noticed a small spot on their eye that gets exaggerated on cameras like when a camera flash makes people eyes look red, this little spot is actually a tale tale sign of a serious disease. After this was told, the influencer went to a doctor and got diagnosed early and they were able to get treatment to reverse a potentially life altering illness. So yes though people should be very careful about speculating about an idols health. But also sometimes the speculation can be helpful, as unfortunately many types of illnesses or conditions are very unknown and rare that sometimes most people can't pick up on it very few know what to look for this includes doctors.

Also, since it's an ongoing long term condition that is managed rather than a physical disease that can actually be cured, something like neurodivergence is also a lot more complex to speculate on than the eye example you mention. If there is a medical concern of any kind with him, hopefully his friends/family/company look out for it and help him through it.
