- In this video, we demonstrate the technique of peripheral access using ultrasound guidance. The video demonstrates the extreme skills of one of NYSORA’s regional anesthesia fellows, Dr Leander Mancel, who used a “creep up” technique to advance the catheter into a small peripheral vein with a caliber of only 3-4 mm. While the ultrasound technology in this case facilitated vein access, we also remarked that traditional methods, like simply applying a tourniquet and waiting for veins to become more prominent, could have been equally effective in this patient, obviating the need for ultrasound altogether. This highlights an essential point: traditional, non-ultrasound IV access remains indispensable and can be even more time-efficient in many clinical settings.

Love them. We used the same device, but it was called Grip-Lok, to help hold neph tubes in place.

Every doctor and nurse should be required to take advanced training on IV placement . Too many patients are literally tortured, by multiple insertion attempts. At Children’s Hospital, Cincinnati , they have a VAT team. For patients that appear to be a difficult stick, they are always called. They are the best of the best. Success first time, every time.

I went through a phase of relying more and more on ultrasound for difficult access and slowly deskilled in normal placement. I eventually realised this after a couple hundred ultrasound placements and tried hard to get back the normal skill. These days i probably only use it for 1 in a 100 patients rather than 1 in 5 when I was more reliant on it. Its certainly helpful for those near impossible patients but too slow in an emergency.

Ultrasound guided IV is a must learn skill for difficult IV access. Out of plane approach, selecting peripheral veins of at least 5mm diameter and not more than 1.5 cm deep, using the M-mode cursor to orient needle to the midline, using long length catheters, advancing needle with “chasing the dot” or Ueda technique are some pointers. In infants & smaller children, ultrasound guided saphenous vein cannulation should be mastered.
M-mode cursor is a good pointer - not often mentioned. Do you always use it? Greetings!
@@nysoravideoM-mode cursor can accurately mark the midline location and be used to guide the needle precisely, use it a lot! Manufacturers should create a midline target box/marker for IV access in ultrasound machines.
