I think it was her mother's, but it could also be her grandma's parents and grandparents, since she met them alive and now they're surely gone.

I hate to tell you this but, the flowers she brought when visiting the grave, has a meaning related to romantic love... so I don’t think it was her mother...

@@maybesoenner grandma Kohaku cried with Hitomi said she wanted to visit her mom, so she did, at the grave. If you still don't believe that side of the theory (which is the correct side), go back to the episode where Yuito meets young Hitomi within the depths of his drawing. young Hitomi draws a black line between a queen and a princess saying that they can't meet and she doesn't know why; that drawing signifies how young Hitomi is separated from her mother
