The TOPXXX chip is shorting and even though I've fixed such chargers, the top chip is always bad.

Hi great job man I wondering if ask u I have change the transistor top 256 en but not the same 1421 because wen I change it won't work for me it just keep blink in red icant find the problem

Great. But the very important question is: WHY did this chip fail in the first place? Poor ventilation? Heatsink too small? Bad circuit design? Shorted out components elsewhere? Why? I don't want to have to keep replacing this chip without knowing the cause of the failure.
