(low amh and ivf ) इस प्रश्न का जवाब विस्तार में देने वाले है ।पिछले videos में हम ने यह discuss किया है की low AMH क्या होता है और low AMH के values क्या होते है। अगर आपके AMH values 1.4 - 1.5 ng/ml से कम है तो आपको Low AMH कहा जाता है। AMH हमे ovarian reserve के बारेमें बताता है Ovary के अंदर अंडे की quantity और quality का अनुमान हमे AMH से प्राप्त होता है।
साधारण स्तिथि में जब महिला 35 साल से कम उम्र की हो, IVF का सफलता दर 35% से 40% होता है। IVF का सफलता दर मापने केलिए सबसे महत्वपूर्ण है महिला की उम्र। जैसे जैसे उम्र बढ़ती जाती है वैसे IVF success rate कम होते जाता है। काफी जगहों पे IVF को लेकर ivf success rate 75% - 80% भी बताया होता है, वह कोई specific category या कुछ महीने की success rate हो सकती है। Low AMH patients की IVF का सफलता दर साधारण सफलता दर से 5% - 8% कम होता है, यानि की सफलता दर 25% - 30% होती है।
Low AMH patients इस दर को कैसे बढ़ाये ?
आप इस चीज़ को लेकर कुछ ज्यादा नहीं कर सकते, जाहिर तौर पर कुछ अच्छे stimulation protocols है। Dual stimulation या Dual Step IVF जो की बोहत ही बेहतरीन stimulation protocols है अगर सही तरीके से किया जाये। Low AMH होने के बावजूद अगर यह stimulations सही तरीके से किये जाये तो यह आपके सफलता के दर को बढ़ाने में कामियाब होंगे। लोग अक्सर यह सोचते है की, क्या donor egg लेने से success rate double हो जायेगा ? पर नहीं ऐसा नहीं होता है आपका सफलता का दर तब भी 35% से 40% ही रहता है।
IVF का पहली बार fail होना काफी साधारण है , बल्कि IVF centers में counselling के दौरान counsellor के माध्यम से यह अक्सर बताया जाता है की patient को 2 से 3 बार IVF कराने की जरूरत हो पड़ सकती है। इस mindset के साथ अगर patient का AMH low है फिर भी consistent और calm रह कर treatment कराया जाये तो सफलता का दर हमेशा ज्यादा होता है।
Low AMH का IVF के साथ यही relation है की success rates बस थोड़े से कम है। अगर patient की stimulation सही है तो उन्हें 25%-30% की success rate मिलती है, provided the treatment is correct. AMH की value कम होने के कारण आपको mentally prepare होना है की आपको एक से अधिक बार treatment की आवशकता हो सकती है। और आपको यह ध्यान में सब कुछ normal होने के बावजूद IVF का success rate normal लोगों में भी 35%-40% से ज्यादा नहीं होता। अगर आप यह चीज़ accept कर लेते हो तो Low AMH होने के बावजूद आप IVF में काफी अच्छा response देंगे।
