    The internal capsule (IC) is a white matter structure in the brain that contains myelinated fiber tracts that connect the cerebral hemispheres to the brainstem, spinal cord, and subcortical structures. It's located in the inferomedial portion of each cerebral hemisphere, between the lenticular nucleus, the thalamus, and the head of the caudate nucleus. The IC contains nearly all information in and out of the cerebral cortex.
    The IC is made up of ascending and descending fiber tracts that pass past the basal ganglia.
    It can be divided into three parts:
    Anterior limb: Between the lenticular nucleus and the head of the caudate nucleus
    Genu: At the junction of the anterior and posterior limbs
    Posterior limb: Between the lenticular nucleus and the thalamus
    The IC also divides the caudate nucleus and thalamus from the putamen and globus pallidus.
    The IC is a unique location where a large number of motor and sensory fibers travel to and from the cortex. Damage to the IC can cause unique findings that allow you to localize the lesions to the internal capsule by exam alone.
    Most internal capsule strokes are not fatal, and most people experience some degree of recovery. An internal capsule stroke can cause arm, hand, leg, or foot weakness, described as hemiparesis or hemiplegia. It may also affect the movement of the muscles in the face, making it difficult to chew, swallow, or speak clearly.
