This is apparently a thing with pitchers, they know they're throwing from flat ground rather than the mound so they have to put a little more height on the throw, but they sometimes overestimate how much extra and just chuck one. Seems like an avoidable problem but it does keep happening
I was 2 minutes in thinking, "Man, idk... Baseball is really boring right now." To that point, they've all been in the realm of, "Man, that sucks, but sometimes these things happen." But that throw at 2:13 is quite horrid, lol
