    Do you think that the LIMA graft in the last case gave side branch proximally, which may cause a steal phenomenon ?

      Very good point. Unclipped intercostal branches may cause coronary ischemia via steal phenomenon, but this is not likely, as the coronary flow is mainly diastolic, while the flow into the intercostal branches is mainly systolic. Also, the steal phenomenon would not cause LIMA atresia. The competitive flow from SVG-diagonal is the likely cause of LIMA atresia here.
