"How do you tremolo pick for that long?" Conditioning, basically; it's all musculature in the end. I taught guitar for 15 years and I'd tell anyone into extreme metal to just grab a guitar and tremelo pick open strings while they were watching TV or whatever, just do it as much and as often as possible to build up the musculature to the point where it becomes a reflex; keep the picking wrist relaxed and just -keep going-, keep it even and don't worry about speed. You'd get a kid turning up for a lesson saying they wanted to play, say, "Funeral In Carpathia" by CoF and I'd tell them yes, you will absolutely be able to play this; but it's going to take at least 18 months of working on your picking technique to build it up to the point where you can reliably pick that sort of thing without developing tendonitis. Build up speed by building up gradually over time; if you try to play fast without warming up and without building up your overall ability, you'll tense up - and that's fatal.
