I generally prefer a grip on the forearm aiming for the wrist. That way if the arm slides, you can usually effectively catch the wrist for control. If you're talking about the underhook, then my aim is to reach for my opponent's shoulder. I don't worry about where his trapped arm is, as long as I can compromise his posture.
Doing a sacrifice throw on concrete or any hard surface is not a good idea. If the throw fails you are compromised. It looks good when it works, but there are many factors involved. In a self defense situation you only have one or two chances of getting a good technique in, and I would never try a sacrifice throw unless it was my only option. The simpler the technique the better.. I have used O Goshi in a fight, and O Soto Gari.. The point is to get away as soon as possible. Sacrifice throws in a real fight on concrete especially is not a good idea.
This is a strange comment. No one is suggesting doing a sacrifice throw on concrete. This is a sport video, and one is expected to be doing these techniques on a mat or some appropriate setting. Some of the techniques could conceivably be applicable to self defense, but I wouldn't presume to call this self-defense or fighting instructions.

At about 2.20 is the guy in white placing his arm at before or after his elbow. With after being on bicep and before being forearm.

Look up the video where this girl in Africa does a sacrifice throw in dirt and actually lands it perfectly
