The guys in white walking around like zombies with less balloons are medicated (medically and/or recreationally). He is told to medicate, but in Leave Me Alone he talked about fearing that if he does he will lose what makes him NF. He feels that numbing the pain doesn’t solve the problems, so he would rather not take the drugs and push on, regardless of how painful it is. In Leave Me Alone he is fighting his inner demons and resisting the temptation to take the drugs. In The Search he is looking for a way forward, not necessarily a solution, but a way to manage his mental health is a positive fashion.

It’s really brilliant because it’s the best way to visually show a negative cloud he’s carrying with him.
