Not sure what caused that. Did you trim off the seeds from the ends of the leaves? Those will often pull the onions over. Did you keep the seed starting mix too wet? It needs to get fairly dry between watering.

I planted 1 bunching onion seed per cell. I kept them watered from the bottom. They grew to be about 2-3 inches then they fell over. Any idea why?
