  • Dr. Ebraheim’s educational animated video describes the anatomy of the anterior compartment of the foot, and the injuries and conditions associated with the ankle.
    There are many structures present at the anterior aspect of the ankle. These structures are often susceptible to injury.
    Common injuries and conditions around the anterior ankle
    1-Anterolateral impingement: Painful limitation of full range of ankle motion due to soft tissue or osseous pathology. Soft tissue thickening commonly seen in athletes with prior trauma that extends into the ankle joint. Tibial bone spur impinging on the talus can become a source of chronic ankle pain and limitation of ankle motion in athletes.
    2-Arthritis of the ankle joint: Commonly the result of a prior injury or inflammation to the ankle joint. Can be usually diagnosed with an examination and x-ray.
    3-Osteochondritis dissecans of the talus: Chip type fracture that usually occurs with severe ankle sprains. Causes pain, swelling, and stiffness of the ankle joint. X-rays, Ct scan or MRI are commonly used for the diagnosis.
    4-Tibialis anterior tendonitis
    5-Anterior tibialis tendonitis: It is an overuse condition common in runners. Common injury that usually accompanies anterior shin splints. If this tendon is strained, pain and tenderness will be felt upon active dorsiflexion or when the tendon is touched.
    There are many structures present at the medial aspect of the ankle. These structures are often susceptible to injury.
    Common injuries and conditions around the medial ankle
    1-Posterior tibial tendonitis or rupture: Posterior tibial tendon problems can occur from overuse activities, degeneration and trauma. The posterior tibial tendon is one of the major supporting structure of the foot. The tendon helps to keep the arch of the foot in its normal position. When there is insufficiency or rupture of the tendon, the arch begins to sag and a flatfoot deformity can occur with associated tight achilles tendon. This occurs distal to the medial malleolus. This area is hypovascular. It presents with painful swelling on the posteromedial aspect of the ankle. Patient is unable to perform a single leg toe raise.
    2-Tarsal tunnel syndrome: It is compression of the tibial nerve in the tarsal tunnel. The flexor retinaculum covers the nerve. Tarsal tunnel syndrome is similar to compression of the median nerve in the carpal tunnel. Causes include ganglia, accessory muscle and soft tissue mass.
    3-Flexor hallucis tendonitis: Pain, swelling, weakness posterior to the medial malleolus. Dorsiflexion of the big toe may be reduced when the ankle is placed in dorsiflexion. Triggering and pain along the tendon sheath may also occur with toe flexion. It often occurs in activities such as ballet dancing, in which plantar flexion is necessary.
    4-Rupture of the deltoid ligament: The deltoid ligament are the primary stabilizers of the ankle joint. The deltoid ligaments provide support to prevent the ankle from everting. An isolated eversion sprain with tear of the deltoid ligaments is a rare injury.
    There are many structures present at the posterior aspect of the ankle. These structures are often susceptible to injury.
    Common injuries and conditions around the posterior ankle
    1-Posterior ankle impingment (os trigonum): It is nonunited piece of accessory bone seen posterior to the talus. It is common among athletes such as ballet dancers. Tenderness in the posterolateral aspect of the ankle posterior to the peroneal tendon especially with passive plantar flexion. May be seen in association with flexor hallucis longus tenosynovitis.
    2-flexor hallucis longus tenosynovitis: It is a condition associated with ballet dancing, in which extreme plantar flexion is necessary. Swelling and pain posterior to the medial malleoulus. Triggering with toe flexion. Dorsiflexion of the big toe is less when the ankle is dorsiflexed.
    3-Achilles tendonitis: Irritation and inflammation due to overuse. Pain, swelling and tears within the tendon. Usually treated with therapy and injection. Do not inject inside the tendon, rarely treated with surgery.
    4-Achilles tendon rupture: Achilles tendon can become prone to rupture with age lack of use, or by aggressive exercise. Rupture is diagnosed by the Thompson test and MRI. Treatment may be conservative without surgery by using a cast or a boot , however the rerupture rate is high. Surgery is done by approximation of the torn ends, however there is a risk of infection, skin and wound complications with surgery.
    There are many structures present at the lateral side of the ankle. These structures are often susceptible to injury.
    Diagnosis of these injuries can be confusing and many of these injuries can be missed.

    Great video. I sprained my ankle real good 15 years ago and now for the last couple of months I've been strengthening the bottom half of my body, hips, legs and now I'm getting into ankles. When doing standing calf raises I get a click directly where an anterolateral inpingment can occur (@ 0:50), During the click there is no pain, but after the workout I get a light dull pain in that area. This is worrisome because I'd really like to train my calves hard (I have a chicken leg complex).

      No. Alterations to that video 100%original. Strata. About 55 seconds in and it seems to shoot or spit. About 4 to 5 times withe the last 2. You can actually. Visualize the spits. Passing the camra lenses ..

    I have a left ankle that swells up and hurts from time to time. Listening to this video it seems like I have a ruptured ligament or tendon. I must say thou the video is a little beyond my education....but I do try thou. lol

    Great explanation. Suffering already for four months and have been told to ask Tylenol for the pain, why is a medical doctor allowed to do that? Anyone older than 5 knows about Tylenol. Now waitinf for an MRI ankle is swollen like an apple.

    my right ankle went "sideways inward" so it looks like -> L if you were looking at my foot from a top view. it hurts all around both sides of my ankle, though I forced my self to walk the next day & since then iv been walking for abt half a month. there is a bump now between where my foot & leg connect but iv managed somehow... it hurts more on the inside of the ankle though.idk

    Combination of soccer and skateboarding I have wrenched and rolled my right ankle roughly 15 times. I recently noticed my ankle pain has gotten horribly worse. Aside from ankle wrenches I get a very sharp burning sensation upon landing on my right foot. Sometimes I feel it comes from my foot being flexed upwards to much but I’m starting to think it’s largely from landing on the ball of My feet. I then feel that burning sensation in my foot and it’ll last for a good 10 minutes. The fact it’s gotten worse has me concerned.

      Permit me to offer a suggestion. Take a towel and grab both ends with hands and put foot in it. Then bend back your foot using the towel. This helps stretch the tendons and ligamants.

    Hi I have had extreme foot pain now for 3 months! I have seen 2 specialist and getting no !! My MRI shows a large lipoma in my arch and ruptured facsia medial and tibeal tendenopathy. I have been in a boot for 3 months and going to PT twice weekly. The dr first said I have tarsal tunnal and changed his mind because my nerve conduction studies were normal. My ankle has some swelling and numbness with shooting pains into my arch and heal. I cant wear shoes because it rubs on the cyst.- help!

    Further, when I do a Wall Ankle Mobility Test, I get some slight pain during the stretch around the posterior tibial tendon. Could the clicking on the other side of the ankle and this pain in the PTT area could somehow be connected to each other?

    Whenever I play football in the sun, it feels like my ankle is overheating, and when I poor cool water on it it instantly stops. Does anyone know what this means? I really need to know. (Just for context I have been playing football (soccer) for 4 years now and I have played for an academy in the premier league for 3 of those years. I now play for 3 teams, and train 3-4 times a week and play 1-3 matches a week. I also sometimes just play for fun on random days.)

    I injured my left ankle outward while playing soccer ⚽, someone passed the ball from the right corner side and I was attempting to score a goal, but unfortunately, the ball only touch my left toe, and I felt so much pain and had to sit on the ground screaming ahhhhhhhh. After 2 minutes I continue to play the game for another 45 minutes with little pain. After that, I gave like one week's rest. And played one full game. And then I decide not to get back to soccer until it get completely better. However, I was going to the gym and doing my regular exercise including running in a trade mill for 20 to 30 minutes. The nature of my pain is I feel uncomfortable and colder stepping or walking on a normal floor without shoes or sucks. It's been more than six months now. I cannot see any swelling. I have been able to go to the gym and perform my leg exercise with slight discomfort. When I am not working out and am less active I feel more discomfort. Sometimes I feel slight discomfort in the other parts of my left injured leg as well which makes me think that it may be related to my ankle injury, I am hoping it will get better on its own is the reason that I haven't talked to the doctor yet. I will highly appreciate your advice. Thank you so much.

    4 weeks ago I slipped and fell. My left ankle hurt, not too bad, but still hurt. I assumed it was a minor sprain and iced it. There wasn't any swelling which I thought was weird. It still hurts when I walk. It especially hurts when I turn my foot to the left. Should I see a doctor?

    the clicking could be your peroneal tendon moving over your ankle bone due to you rupturing the superior retinaculum....definitely see a doctor.

      Push yourself to your limits dont use a brace or crutches if you have them and start walking jogging and running as soon as you can if you can take the pain push through it and you will heal in no time

    They clearly had someone narrating off the slides. Quite a few words were mispronounced. Posterior, paresthesia, differential, Tinel--these were the ones I heard. It actually makes the video fairly unwatchable, since you're listening to a voice which clearly is not familiar or comfortable with the source material. Kind of a dumb way to produce a video. There is a certain "something" in hearing a voice that knows that it is talking about. You can hear it easily when it's just a rote narration.
