Looks great! I assume that the fly rides hook and wing case down.

So how is this fly fished? In the surface film with the hackle on top, or subsurface with the hackle representing legs?

Great question, Ideally this fly pattern should be fished with the hackle side down. Also, The hook eye should face down, instead of up, but I did not have a dry fly hook like that.
