@@gamernoob5516 inside him, because of her his organs were damage, but because of an accident where a thing which I forgot the name fell on them, the ghoul die and he survived but not so long because of the damaged organs. So since th ghoul organs where in good condition and to save him the doctors transplant then to him making him a ghoul. ( or something like that, long time I stopped watching)
