Ideally, all the surfaces would be free of airspace obstructions. You are correct. Topographic features often penetrate the conical surface, horizontal surface and even transition surface in some cases. Generally, the most critical surfaces to safe aircraft operations are the primary, approach and transitional. Please note, penetrations of the horizontal surface can result in adverse impacts to airport operations.
