Thank you Dr weiner more doctors need to see this, I too have noticed that fillers that have been in place for many months were hard to dissolve...do you inject more hyaluronidase to dissolve those old fillers or do you tell the patient to come for another visit? And another question how long is hyaluronidase valid for use after reconstitution because some colleagues inject it a few months after reconstitution and they report it working well dissolving fillers

What do you think it’s the best approach to dissolve under eye acid filler (Tyndall effect, injected too superficially)? one 20-25 units per side, 1 single round or several microdosis of 5-10 units per eye. I’ve read that many people have had bad results after having their under eye filler dissolved, they report volume loss and hollowness. Also applying more units on one side could create asymmetry? Please your opinion would be very helpful
The amount of Hylenex is dependent on the amount of filler needing to be dissolved, the duration of the filler, and the amount of crosslinking of the filler. I don't believe there is any issue with permanent changes from hyaluronidase and there is nothing in the literature (that I know about) to support that. The filler sometimes forms a capsule which requires a needle to enter. Ultrasound is useful often for these cases to localize and characterize. Micro amounts of hyaluronidase makes no sense and not recommended. Multiple injections is related to inadequate dosing or not exact placement in addition to the crosslinking. Remember Juvederm and Vycross are hard to dissolve.
Dr Weiner, one more question if I may. Is it possible to remove some or all of the filler from the skin if it was injected there 1-2 weeks ago and didn't have much time to "fix" inside the tissue?

I have been trying to dissolve my volbella under my eye between hylenex sessions and months of waiting in between. I wish I could know what is a good dose of hylenex to work with . I have done 10 units three different sessions , then 30 units and then 8 units each session per eye. No change. Please help

It's too complicated to respond here. Antibiotics isn't necessarily the first mode of treatment. There are several papers on the net and there isn't agreement on treatment.
Hello Dr. Weiner. I had 1650 units of hyaluronidase but still have a filler in my chin which is calcium based for 3 years. How long does it take to completely dissolve? It is better to wait or to take more hyaluronidase? I also had a nerve injury from injection. Thank you.
@@StevenWeiner I had a Neauvia intense rose. They said it is HA based and hyaluronidase can be used to dissolve it but as I researched hyaluronidase just disperse HA. I hope it will go away as soon as possible, it’s been threes years since I got HA. Thank you kindly Dr. Weiner for your response.
@@StevenWeiner It is Swiss brand. Thank you for your honesty. Some dermatologists are not asking you what kind of HA do you have before they gave you hyaluronidase. It is getting better after 4 yrs but it stayed much more longer than it is predicted and now I have some redness at this place. Hope redness will go away soon as possible too.

I use more hyaluronidase for older filler and do more repeated injections. We don’t have the hyaluronidase that needs mixing.

Radiesse is not dissolved with hyaluronidase or any other substance such as STS. You can try mechanically breaking up with needle or cannula.
