The items don't go away, the shells are what it shows. If there is 3 live 3 blank, there are 6 rounds in the shotgun. Changing the polarity doesn't remove or add a shell. It's turns blank live or live blank. Drinking a beer is the only thing that I can think of at this moment that racks a round. You can shoot a live, blank, or drink a beer. You keep miscounting, thinking things are changing when they aren't. The odds aren't that bad.
The "Count" resets upon the bots death, or shells run out. That's when you're also given new items. The count doesn't carry over and over and over, unless you mod it to if there is such a mod. The count is whatever it shows from showing the shells, till either you/the bot dies, or it runs out of ammo, and new shells come up.
