This effectively becomes a third blind and he has the option to raise if it is limped to him. Some games have a straddle on the button where the pre-flop action starts on the small blind. I have mostly seen this in Pot Limit Omaha.
Considering the QQ four-bet pre-flop, I'd agree, she could have gotten away from it. Now, If that guy FLAT-CALLED with QQ to her 3-bet, yes, she would have gotten felted on that run-out

Yeah, turn should be a fold. Tarzia maybe not bluffing much preflop there, like all the other live game fish regs, but all 16 AK combos are defo in his 4b range COvsSB. Decent regs ranges prolly consist all AK, QQ+ and A2s-A5s there. Flop discounts some of the bluffs that now have the nut flush draw, so even if he shoves AcKc and the low suited club Aces, he still has 18 Combos of QQ+, which makes Abernathys equity drop to around 15% or so. Looks like a call at first sight, but it just has to be a laydown against a player that rarely has a bluff there.
