The middle hole is for unscrewing, the end you're using it to clean the steam wand. If you continue unscrewing it like that the tool will break
You can also use the opening hole in that tool to screw and unscrew as well. I just recently found that out as well. Who would have thought! Thank goodness for the internet! Haha!
