  • Penile rehabilitation is crucial when it comes to prostate cancer treatment. In the world of medical secrets and unspoken concerns, there exists a crucial strategy that many doctors keep hushed when it comes to prostate cancer patients - "penile rehabilitation." In this enlightening video on UroChannel, Dr. med. Dr. phil. Stefan Buntrock, a urologist from Göttingen will uncover why this topic remains shrouded in taboo and how it can significantly impact your life if you or a loved one face prostate cancer.
Prostate cancer is alarmingly common, with nearly 300,000 cases diagnosed in the US alone in 2023. The truth is, the majority of patients are left in the dark about penile rehabilitation, and this ignorance can lead to permanent damage to their sexual function.
Prostate cancer treatments like surgery and radiation therapy threaten your erectile function by damaging the crucial erectile nerves. The body can regenerate, but nerves take time - often months to years. Even if surgery spares these nerves, they remain sensitive, easily susceptible to damage. This leads to a lack of erections, penile fibrosis, and even venous leakage, resulting in long-term damage.
Enter penile rehabilitation - a critical strategy that seeks to counteract these consequences. While there are no clear guidelines, one widely accepted method is the use of PDE-5 inhibitors like Tadalafil, administered early in the diagnosis process to maintain tissue function.
This strategy is not only relevant for surgery but also for radiation therapy. Evidence suggests that long-term use of medications like Viagra and Cialis can protect erectile function. Vacuum pumps, injection therapy, and regenerative therapies like shockwaves, platelet-rich plasma, and stem cells also offer potential solutions, each with its own set of considerations and outcomes.
This video is a glimpse into the world of penile rehabilitation, shedding light on a vital aspect of prostate cancer treatment that every patient should be aware of. Remember, it's not medical advice, and consulting your doctor remains crucial. Don't miss out on this essential knowledge - your sexual health and well-being may depend on it.

My surgeon said I was not going to like using a pump. I learned that he had never tried one, but was repeating what his patients told him. We got him a pump and one of our nurses trained him on using it. He ended the lesson within ten seconds and said he would never do that again.

So the shrinkage after the prostate surgery is not from the surgery itself. Its from the fibrosis that will occur without nighttime erections?

No, right after surgery is different. We think it could be some kind of nerve reaction to surgery. The penis only appears smaller. But real change will take about a year and will progress very slowly.

This is such an important message. We have found that some men give up months post surgery. It's like a switch flipped in their head that their sexual like has ended. I know because I was there, trying to accept that my sexual life had ended. I interviewed a man in a rehabilitation trial who would not bring up the possibility of sex to his wife even if his rehabilitation was successful because she had to accept that sex had ended in order to help him accept it. He felt trapped and helpless, ending therapy in about a week without a follow-up interview.
