  • Do you know what hemodialysis is? Do you know anyone who does this therapy?
I'm Antônio Laender .. general practitioner and nephrologist.
    Do you remember Anastasia?
Pedro decided to take his mother Anastácia to the hospital. She has had high blood pressure and diabetes for many years, and for some weeks now she has been feeling sick and lacking in appetite. In addition, Pedro realized that for the past few days she has been sleeping sitting because when she lies down she feels short of breath. He also noticed that Anastasia has been a little confused.
The doctor examined and found that Anastasia was swollen and with uncontrolled pressure. She then measured her glucose ... but her glucose was good. The doctor noticed that Anastácia's skin had scratched marks. Pedro stressed that his mother scratches her body a lot. The doctor decided to order some blood tests.
Minutes later ... the doctor returns with the results. The exams are bad.. the dosage of creatinine, urea, potassium and phosphorus are very high. Anastasia's kidneys cannot keep these toxins at BALANCED and SAFE levels in the blood. She is going to have to start hemodialysis today.
    Hemodialysis?
Take it easy! Hemodialysis is not a death sentence.
Hemodialysis is a form of RRT, that is, renal replacement therapy.
Pedro, your mother has chronic kidney failure. The doctor reported that Anastasia has complications of the chronic form of kidney disease, such as anemia and increased parathyroid hormone. This elevated hormone causes diseased bones and itchiness throughout the body. Anastasia's kidneys, over the years, progressively went bankrupt.
Although Anastácia is urinating, her kidneys are not filtering the blood well. Therefore, the blood is getting dirty with impurities (the most known are urea and creatinine) and dirty with electrolytes (the best known are potassium and phosphorus). That is why people who have poorly functioning kidneys have elevated dosages of these toxins in their blood and there is also accumulation of water and sodium in the body causing swelling and uncontrolled pressure.
There are 2 forms of renal replacement therapy or RRT:
    Dialysis and kidney transplantation
Both are forms of treatment that replace the impaired function of the kidneys. Such therapies do not cure the kidneys ... they only provide support, that is, they serve as crutches for sick kidneys.
There are 2 types of dialysis: hemodialysis and peritoneal dialysis
In short, hemodialysis is the therapy that is performed in a hospital or clinic 3x/week while peritoneal dialysis is the therapy that is performed at home every day.
The doctor said: Pedro, when it comes to the urgency of your mother's clinical condition, we will have to start hemodialysis today. But I would like you to visit me another day to talk about the OTHER kidney replacement therapy options in detail.
Anastácia then underwent a small surgical procedure to implant a 2-lumen catheter.
The 2-lumen catheter can be implanted in a deep vein in the neck or in a deep vein in the groin area.
Through this catheter, blood is drawn, through one of the lumens, by a machine and travels along a circuit in order to pass through a filter (also known as a capillary or dialyzer). This filter makes the blood balanced in the content of impurities and water. Then, the clean blood is returned by the machine through the other lumen of the catheter. Each hemodialysis session lasts an average of 4 hours and happens 3x/week.
And in the next videos you will find out more details about kidney replacement therapy options! Have a nice day!
