After passing the plab 2 you have to secure a job in the NHS. Only after you have passed their interviews, pre-employment checks and obtained the correct visa then only do you start a job. And indeed once you are an employee you will be paid for your services. It's quite an extensive process after passing the plab 2 of securing a job.
