  • Braces Explained: How Teeth Move / Orthodontic Tooth Movement:
    (Special shout-out to Luka Andjelov for the video idea!)
    In this video, I will be answer the question of how braces, invisalign, spacers, etc are able to move your teeth! As orthodontist, we specialize in idealizing your smile and to do that we have to be able to control where the teeth move and how they fit together.
    Contrary to what you may think, teeth are not fused to your bone, and a bit of movement is normal. This is due to the fact that teeth are suspeneded in your bone by millions of fibers called periodontal ligaments, or PDL, for short. When the PDL is compressed by force, the bone on that side will resorb, or eat away, and on the opposite side, where the PDL is stretched, new bone will be formed. This is what makes teeth move through bone. In this video, I will show two cases of how teeth can be moved by applying a slight pressure & tension to the PDL and activate bone cells to allow tooth movement.

      I've already commented this but in case it gets lost in the hundreds of comments, I'm saying it again lol:
      How far can teeth be moved? Can they be moved anywhere in the mouth?
      My upper row is not aligned and balanced. The right half of the upper row is too low and the left half too high. It's hard to explain without a picture but let's just say that when I bite into a straight stick, it isn't level. It isn't aligned to the rest of my face and this is because of the teeth being in the wrong place. The entire upper row needs to be shifted to the right and slightly up on the right side.
      So my question is, would that be possible if the teeth need to be moved a considerable amount before they are in their right place?

      Teeth can be moved very long distances but it may take a long time and result in root resorption. I would suggest you see an orthodontist regarding this concern. It can be fixed but if it is too severe sometimes jaw surgery may be necessary

      Hi Doc, how do I know if my gums are strong enough to handle the pressure that braces puts on the teeth? Also, can braces cause gum recession?

      Dionna ly YES! I had spacers and rubber bands. The rubber bands would pull my mouth shut to the teeth that already had tons of pressure from spacers, It was the worst pain and I remember crying from it the first week. Mouth pain is the worst, I handled contractions better than pain from braces.

      EL PASA it’s a different type of pain it’s more like pressure like someone is pushing on your teeth with a lot of pressure. And it makes your teeth kinda sore

      I have a really bad overbite, I mean my front two teeth are a way too forward and it causes me a bad overbite ..the thing is, after my overbite gets straightened wouldn't the space inside my mouth get less? I have heard that if the tongue doesn't get enough space to rest in then it will cause me breathing problems? Is that so?
      My front two teeth and also the lower two/three teeth are sticking out badly, I am not scared about getting braces or even having some extractions done..I am getting stressed because of the less space it will create afterwards..I can't really close my mouth because of my teeth and it's embarrassing!

      @@sneha_m09 same thing happened with me....I have my braces for 3 freaking year.....and now I feel that my tounge have not enough spaces to rest in mouth....I really feel worried.....when recently I talked to orthodontist...he told me that...this is called "tounge thrust" and u must have this problem from your childhood....and further he told that I have to wear appliances for sometime which u can see in UA-cam by searching appliance for thung thrust....but the problem is this appliance can create spaces between my teeth....so for that again I have to wear braces....now I really don't know what to do....
      If u found any solution for this...please let me know....

    I have braces because I’ve been so insecure about my TMJ. I have a nasty overbite/overjet and I currently have rubber bands. Nice to know the pain is so worth it.

    You’ve answered so many of my questions. Thank you for the amazing visual explanation. I wish my orthodontist would’ve sat me down and done something similar to this in the beginning. I am currently in month 6 of my treatment.. I’ve seen amazing progress with how straight my teeth are but barely any with my deep overbite. At the moment I only wear elastics overnight.     Thank you! This is a very good video! I have a front tooth that is longer than its neighbour and I had no expectation that braces would be able to solve that. Imagine my surprise that that issue has been resolved after just two months with braces! I came in search of a video that would explain exactly how that happened, and now I know.

    This was so helpful. Perfectly explained. Thank you so much! I’ve had my braces for about 2 years now, and I just recently noticed my bottom sets of teeth wiggling. I freaked out because I have short roots on my two front teeth. I asked my ortho to show me an Xray before my braces in comparison to now. I saw that my roots were still the same length, but it still freaks me out that all of my bottom teeth are wiggling! Watching this video put me at ease. Thank you so much!!

    Hi Dr.
    I'm confused😕 not by you but byothers orthodontist results that i see online. And the (?) is ; There is a " perfect " position where the tooth is supposed to be? like if there is a guideline that orthodontist follow with the skull? Because i see some results from different orthodontist and it seems that there still more work to be done. Some cases i see straight teeth but face recession. Sometimes overbite, i feel like 'the after' result still have an overbite.
    Hope u can help me with the confusion and btw i learned a lot with ur videos, thank u for the time teaching, they are really helpful.

    Hello Doctor, I got my first premolars extracted from both upper and lower jaw and from both side too, its been 7 months and I noticed that my gap on my right side is filling very slowly. What should I do to fill extracted teeth gap faster?

      Hi Shilpa! That seems about the right amount of time for your teeth to move to close an extraction site ! Just be sure to not eat any of the foods that you shouldnt so that the brackets dont break & keep your teeth clean! Your orthodontist will do the rest =] Keep me posted on your progress!

    Why do some people get bands and others don’t when they first get braces?

      Hi Sean, Great question. Some orthodontists use bands on the back teeth while others use brackets so it is more of a case-by-case basis depending on your orthodontist. It does not significantly effect your final product - there are pros and cons of each =]

    I’ve heard that you shouldn’t take advil/tylenol before you get your braces on/ tightened because even if it will keep pain at bay, it stops your teeth from moving or something.. is this true? what pain killers do you recomend?

      Advil actually has a MINOR effect of slowing down tooth movement at first. I usually recommend Tylenol because it causes relief and doesn't have an impact on tooth movement. Great question!

    I love this!! Thanks for teaching us about what we can’t see as patients. I’ve been documenting my progress on my channel so it’s cool to see the “behind the scenes” in regards to the pressure points. Can you help me with why my molars have been moving forward and backward? My spaces can’t seem to fully close and when they do, they’ll retract back. It’s the reason my treatment has taken longer than expected.

      It depends on what kind of a fake tooth. For example, implants can't be moved but if you have a crown, you can move it

    I got top braces only and go back to get bottoms in 6 weeks. The braces dont hurt like i expected and i dont really feel any pressure.. Are my top braces even doing any work these first 6 weeks ?

      Hi Shay! We aim to begin with super light wires that put a light force which don't cause much discomfort. Did the orthodontist put in a wire that connects the teeth or is it only brackets? If there is a wire, the teeth will be starting to move!

      Shay H i just got my braces today and they only did the top, I was like can we get this whole thing done lol but the pain I feel I’m glad I don’t have both right now. I have the wire in mine and go back in 7 weeks

      I got both top and bottom done on the same day three months ago. I'm starting to notice a difference! They're really not that bad except when I go back to tighten them. It only hurts for a day or so.

      XtRemE cLasHerS it does not hurt when they are tightening it. They just change the wire. For a few days afterwards they might be sore

    If a tooth is basically in the right spot, but needs to be rotated, how does that happen? As opposed to the tooth moving “through” the bone to relocate.
    I am 44 and about to have braces for the first time, I’m excited and terrified. 😂

      I just got my Hawley retainers and am trying to figure the over production of saliva it’s so annoying am scared to open my mouth. Lucky though with the pandemic I can just wear a mask to cover any pending embarrassing accidents.

    So, my question is why do teeth want to move back after completing braces or Invisalign and we need to where a retainer? I'm thinking the new bone that has grown is soft but what exactly is the force pulling the teeth back?

      Yupp! Check out my video on retainers. The forces that push the teeth back are the constant battle between remodeling of your bone, forces of the cheek and tongue. Retainer prevents these forces from moving the teeth!

      After braces you do need a retainer for life, or as I like to say it "as long as you want your teeth straight"!

      I've had top braces on for 5 months now. Some very brief relief to me is to drink cold water and have it run over the teeth that are hurting over and over again. I find it very soothing. If it's especially unbearable and hurting constantly even when you aren't putting pressure on any teeth, ibuprofen should bring the pain down.

      one tooth hurts because it’s moving form, (it’s place) , my teeth also ended up looking worse than i started , but in the last 3 years i learned that they get worse for structure , so ur comment was 3 months ago, meaning most likely the fact that it looked even worse had something to do with straightening your teeth .

      Sometimes when braces are put on, the initial wire creates some spaces between teeth but don't worry! I'm sure your doc will fix all the gaps and make sure your teeth look excellent once you're done!

      Immediately after braces there is a high chance of relapse as the PDL is very stretched out. This is why it is imperative that you wear your retainer to prevent relapse! =]

      I am currently wearing retainers and sadly ALL retainers has to be worn for life. I wear mine at night for 8 - 10 hours. If you dont wear your retainers, your teeth will naturally shift back to its comforting and natural position.
