First death. For her second and permanent death she was pushed off by Cesar Faison for his first return from the dead storyline. He was in cahoots with Helena and up to all sorts of shenanigans. If I remember correctly it was Helena who wanted Katherine dead for carrying Nicholas’s baby and intending on marrying him. Helena wanted none of that and Cesar murdered Katherine for her.
