That's why I love ds2. There are dozens of moves and animations that you're statistically unlikely to ever see because they have such a miniscule chance of happening.

that happened to me. It only works if you're dealing chip damage and you're very close to him. I dunno if it's really the case, but that's how it happened to me all the time.
Same with Margit, that's how you can see DS2's influence on Elden Ring. Fought Margit about 20 times now and he whips out a completely new move every other fight. Maybe new ones are added in every patch

the thing I hate about Dark Souls 2 you can't feel the weight of the animations everyone hit like some low budget movie actor

Imo DS1 and DeS are the best ones in this area, you can even feel the weight of an armor
They should buff that damage by 100%
