    As a 30 year gout sufferer (71 YO), I found the following:
    Gout can be in any joint of the body - however most prevalent in the big toe.
    I had one attack in neck, many others in various joints.
    MD treatment and pharma meds on a daily basis did not stop my gout attacks. Gave up this strategy after about 3 years.
    Avoiding high purine foods and alcohol did not stop my attacks.
    Exercise and natural remedies helped more than any of the above. My daily cocktail was and still is:
    Dark cherry juice, turmeric, ACV, celery seed, fresh lemon juice w/pulp, cinnamon, black pepper.
    Even with the above cocktail the attacks become more frequent with age.
    Keto diet stopped all pain and swelling in 10 days.
    After that I switched from Keto to High Fat/Low Carb diet.
    Now, after 4 months, gout and related symptoms are still gone.
    I eat many foods that are on the DO NOT EAT if you have gout lists. Asparagus, mushrooms, pork, bacon, red meats, etc.
    Still drink some beer but in lesser amounts and it's low carb.
    Weight has decreased by 13 lbs. (now at ideal weight for my height)
    Blood pressure has decreased from ~150/78 to 120/71.
    Sleep is much better.....
    SUGAR is the ENEMY.

      thanks, im just diagnosed with gout, and I im trying to track down the culprit and I can swear that my last gout hit came after binging on halloween candy. Iv been conflicted as to the safety of a ketogenic diet for gout, your comment reassures me. my n=1 results will tell...

    I’ve suffered from gout for the last 6 years with about 1 attack every 1-2 years. Last year I had both kidney stones and gout attacks. My checkup demonstrated I was diabetic with A1c of 7.7 and many of the metabolic syndrome symptoms like inflammation low energy etc. after the test I immediately adopted to a keto and IF diet with regular exercise. In a month I’ve reversed many of the inflammation symptoms lost 15 lbs (from 224 to 209) and feel great. I’m not cured yet but well on my way. I’ve very studious and doing daily research on the diet and I’m convinced it’s the way to go for me. Today I woke up with the first signs of a moderate flare up. This is how I found this video. I had been warned that Keto increases Uric acid UA so I’m very watchful for symptoms.
    Between the things I’ve read/seen was the notion that UA can act as an anti-oxidant in the body. Yesterday I consumed a high level of anti-oxidant supplement as I thought I may have been exposed to COVID. It was a false alarm. However this leads me to tie the two things together. If I consumed more anti-oxidant supplements than normal maybe this reduced my body’s utilization of UA for the same purpose? It’s just a hypothesis that I want to share to see if it rings a bell.

    Gout for 24 years...ketovoir for 2 years...it works ! Wish I'd done it years ago...all along docs were telling me to eat the wrong things...red meat is not the enemy, sugar is...

    We know that fasting and vigorous exercise raise our serum uric acid, but there is controversy regarding why. Some doctors say that ketones compete with uric acid for expulsion in the kidneys. Others say that excess UA is a waste product of catabolic fat burning. Most say that hyperuricemia is a metabolic dysfunction caused by a genetic defect. The Egyptian Ebers Papyrus (1500 BC) recommended colchicine for gout, as did Hippocrates (470-360 BC). Antonie van Leeuwenhoek (1632-1723) was the first to see urate crystals from gouty joints in 1679. Dr. Thomas Sydenham (1624-1689) suffered from gout, and he wrote extensively about it. One fact that most doctors don’t know is that our gut biome consumes and transforms fully a third of our total UA, and expels it in the feces. For now, most rheumatologists simply tell you to take drugs every day for the rest of your life (e.g. allopurinol). I refuse to do that. There must be another way.

      @@Heisenberg991-m1i Suggestions. [1] Don’t worry about “keto.” Just eliminate all sugars and processed foods. If you want to lose weight, also eliminate all breads and all starches (rice, pasta, potatoes, etc.) [2] Coffee has xanthine oxidase inhibitors that help control gout in many people. It helps me. Coffee may help you. (No sugar!) Plus, coffee is a diuretic that makes you urinate a lot, which helps. At this point the only liquids I drink are water and coffee, with cream. Celery too has xanthine oxidase inhibitors. [3] Spinach triggers gout in me, and in some other people. Learn your personal trigger foods, and avoid them. Everyone is different. [4] Drink more coffee. (Yes I repeated that. No sugar!) [5] Don’t be afraid to salt your food. This will not hurt you. It will only make you thirsty for water, which is good for gout. [6] You may need to ease up on fasting. Just eat smaller portions, and eat less frequently. Weight loss is good, but controlling gout is priority #1. If you respond to this, I will return with more suggestions.

      ı need people to talk who has much experience on that case..
      pls enlighten me ıf you can in my case,
      ı am bit confused what to do..
      ı am 41 years old 1 month in keto resulted evelated ur like 11.7..
      ı just one gout attack 8 months ago triggered by covid 19 medication ı guess.
      so ı found out my uric acid level is high..
      it was 7.9 in the attack ..
      anyway due to this and prehypertension like 13/80 ,visited the doc again and found out ur its 8.8.so
      ı started to keto becouse ı was consuming too much carbohydrates and not feeling well ı knew that ı have inflammation and metabolic sendrom..
      before keto ı was feeling uncomfortableness at my toe.
      after keto it faded away and ı feel totally better.
      drinking atleast 3 litres liquid ,less then 50 grams on carbs a day.by the way ı am in ketosis now for sure as my test says yesterday 3+ ketons.
      coffee ı am bit worried about hypertension!!
      my body fat is %15 doing 3 days weight lifting and 2 hiit cardios and normal intensity walking 3times a week:)lol
      should ı decrease this?
      and considering my sitiution should ı go on with keto which ı am decided to do..

    My most recent lab test puts me at 8.3 mg/dL. Right at the top of the range. I've got nearly two years into the keto lifestyle and eat about 13 grams of table salt per day. I tried cherry extract and it spiked my blood sugar.

    My experience. First diagnosed with gout at age 24, now 57. Lab uric acid levels always high in blood and urine. Frequency, duration and severity of gout flares in various joints increased over the years (big toe, knee, Achilles tendon, finger, wrist, olecranon bursa sac covering the point of the elbow). Frequency and duration of gout attacks precluded establishment of consistent exercise habits. Allopurinol dosage increased from 100 to 300 to 600 mg/day with little to no effect. About 18 months ago I started the Keto diet on my own research and initiative for reasons having nothing to do with gout (I was seeking a sustainable weight loss solution to stave off an indication of pre-diabetes). My doctor had just mentioned the possibility of adding daily Metaformin to my Allopurinol. Doctor never mentioned the ketogenic diet as a means of losing weight and/or preventing diabetes. Doctor was more focused on adding another pill to my daily regimen.
    Over the course of a couple months I lost 30 pounds (6 feet tall , went from 245 down to 215) and realized my gout just unexpectedly fixed itself and completely went away. I also quit taking Allopurinol when I realized the gout attacks had gone away. It has been 18 months of utter bliss with no gout attacks. My blood pressure is great and my lab results are all in the normal range (uric acid is borderline on the high side of normal). I take no daily medications for anything. Keto diet was a very easy transition for me.
    The hardest part of the keto diet is it is can be really expensive and inconvenient to maintain when travelling and/or eating on the run as there are little to no economical fast food options. It is also sometimes hard to fly under the radar in mixed social gatherings (pre-covid) serving food. There is always some self-appointed nutrition expert that bird dogs the culinary choices on your plate and proceeds to publically out you and shame you in a microaggression by trying to make your diet a topic of public conversation. There is always at least one in every crowd. Even well meaning people will offer unsolicited compliments on your weight loss and then proceed to pepper you with follow on questions seeking to learn all the details on how you did it. The whole topic just seems to polarize and aggravate certain people. An honest good faith explanation of your reasons and success just seems to make matters worse. Whatever you say is dismissed out of hand and goes in one ear and out other by the self-appointed nutrition experts that are invariably triggered by such things. On more than one occasion I have found myself on the receiving end of an uninvited public lecture about how the keto diet is a dangerous fad diet trading short term weight loss for an early grave (heart disease, cancer, etc), blah, blah, blah.

      @@owenthomas9863 I was experiencing frequent gout flares and my uric acid was high when I first went on allopurinol at 100 mg/day. I stuck with it for several months then quit on my own initiative because it seemed like it was doing absolutely nothing to reduce the frequency or severity of gout flares. Several years later, a new doctor first informed me that allopurinol dosage ranges from 100 mg/day to 800 mg/day. He said the minimum 100 mg/day was way too low a dosage for my body weight (240 lbs). Allopurinol dosage comes in 100 mg and 300 mg tabs. He suggested I give allopurinol another try at 300 mg/day. I did pretty well at 300 mg/day for several years. Gout flares did not disappear, but the frequency and intensity was reduced. After a couple years at 300 mg/day the gout flares began to increase in frequency and intensity. Upon discussion with doctor and confirming lab blood test results showing high uric acid the daily dosage was doubled to 600 mg/day (two tabs/day). 600 mg/day seemed to work for a couple years when gout flares, once again, began to increase in frequency and intensity. My lab blood test results have always been high and gets flagged as such on the lab report. I would usually only get a blood test when my doctor was renewing my annual allopurinol prescription (as a means for the doctor to justify renewing the prescription). Over the years I also had two 24 hour urine tests in conjunction with a blood test. The urine tests confirmed uric acid was high in both blood and urine.
      About 2 years ago, my doctor upped the allopurinol dosage to the maximum 800 mg/day. However, I never actually started the higher dosage. Coincidentally, I started the ketogenic diet for reasons having nothing to do with gout. I had a blood test showing an indication of pre-diabetes. My doctor was eager to get me started on another daily pill (Metaformin) to manage pre-diabetes. I discovered and researched the ketogenic diet on my own initiative as a means of getting my blood sugar under control. Doctor never once mentioned the ketogenic diet as a means of managing pre-diabetes and/or gout. After a couple months on the ketogenic diet I realized my 30 plus years of gout flares had completely gone away. I stopped taking allopurinol on my own initiative was pleased and relieved to see my gout flares did not return. I had one uric acid blood test back in December 2019 after being on the ketogenic diet for about 5 months. The results were on the very high side of the normal range, which for me was the lowest test result I had ever known.

      @@petertownsend252 Thats amazing. Its seems in your case the allopurinol lost its effectiveness over time and was only a short term solution. Lots of people are saying Keto has done wonders. Im starting too suspect wheat/grain type foods as the cause of our joint problems. Iv havent gone full keto yet ( really difficult for me atm) But have eliminated all sugars and reduce bread alot. And havent had a flare up since (3months).

    My first gout attack started 31st March 2021, and it's getting worse. Today, went to the doctor to get a blood test. On the way, I almost fainted from the immeasurable pain. I wish I had stuck to Keto since 2012. I believe the real cause is insulin resistance. Sugar seems to flare up the gout the next day.
    I'm 27 btw.

      I know how the pain feels. I think there is an advantage to be ketogenic but there is also risk of a flare up in the beginning months.     I started my ketogenic diet with a uric acid of 10.5 and three months after starting I increased to 11.7. I'm going to keep testing to see if it goes down. I started drinking 3 liters of water with 10 g of sodium chloride

    Thank you for the analytic and scientific post.
    I am having literally now the gout attack.
    I started keto 8 years ago and I am on the carnivoreish for 2 years. I am also the high UA levels maybe more than the 20 years and the very first year of keto, I had the gout attacks twice. The first one was very light but the second one was the real one. Since then, I didn't have gout attacks but my UA level was like 7 to 8mg/dl from the annual check result. If I recall my memory, think I had the big toe injuries twice before those of gout attacks. However, this current one, I do not think so. So I agree with you that some inflammatory event triggers the gout attack.
    By the way, the gout graph after the HIT, like Dogh McGuf's?, did not increase too drastically was very much intesesting since such protein breaking down exercise was one of the suspect .

      My uric acid spiked in the early days/months of the keto diet but has settled out at 8 mg/dL now. I eat 13 grams table salt per day between food and drink. No attacks in recent times.

      @@DrPetesKetoKlub Thank you. I take salt like 5g a day therefore I will try more. I also started taking magnesium and pottasium too. After the spring attack, now I haven't got one even I had the liver and heart meat. I will continue to watch my conditions.

    You need carbs in your diet, keto is not sustainable. That's why I tried the *Agoge diet* and it worked great for me.

      Keto does not eliminate carbs. Vegetables are carbs, for example.

    Might the higher UA level in the morning be tied to less excretion by urination due to being a sleep for ~ eight hours, then as you hydrate and urinate thru the day UA levels drop ?

    Thank you for this analysis. Your symptoms are identical to mine. Religious adherence to a Keto lifestyle but crushing gout attack at presence. Nothing I eat is a recognised gout causer. In fact I have cut out foods accepted on Keto but known to be gout causing eg certain oily fish, shellfish and nightshade family vegetables. I wish I knew what I could do. It’s beyond frustrating

      Try a varied carnivore, it cleared my chronic inflammation in less than a week.

      I've realised that the 'normal' I felt was actually a state of having become accustomed to a level of discomfort, we shouldn't have constant stomach bloating and pain, even background.
      Only non animal products I can eat with no payback is berries and onions, everything else has a price.
      I suffered for years with constipation, nothing for days then bathroom battles, now I'm regular as clockwork and it's pain free.
      People do not realise that daily discomfort is NOT a normal part of digestion.

And I think when you go keto inflammation comes down because of carb elimination but then when you go carnivore there is more elimination of plant based inflammatory nutrients. Whenever my wife goes out of town I eat less vegetables and more meat. I'm with you all the way!

    Did not work for me. Keto or carnivore. My gallbladder screamed at me and wrecked havoc on my system. Gout attacks and all. The only diet that works for me is 35% carbs 40% protein 25% fat

      Congratulations! I think we all have to find the diet that works. I think it is notable that your carbs are lower than the standard American diet. I'm guessing that you have cut out sugar, sugary drinks, and alcohol?

      @@DrPetesKetoKlub that is correct and I do like having a lower carb intake. My body don't like a lot of carbs. And I definitely don't take any sugar except from a few fruits I might eat. And I don't drink anyways.

    Of all the video i seen, none of them talk about the water we take daily. Its the water dude. Why do we take so much vitamins nowdays? Because we are drinking reverse osmosis dead water without mineral in it.

      Actually I talk about water in a number of my videos. You are right, water is important. I recommend about 3 liters per day plus or minus.

    But feel amazing since on keto way way less joint pain cut out carbs and sugar all sugar even fruit 4 mos now on keto my MD Dr who I think is an amazing person unfortunately they are just not trained in gout once you the sugar all forms of sneaky sugar clean keto is not like the old Atkins from the 80s Good health and life is all about controlling blood sugar.     Can any one tell me?
    1 month on keto uric acid levels is elevated from 8.8 to 11.7 !!!
    What to do?

      It is normal for uric acid to rise when you start the ketogenic diet. This happens because the ketones that you are producing block the excretion of uric acid in the kidneys. After your body begins to normalize to the low carb high fat lifestyle the uric acid will come back down. Talk to your doctor and make sure you have anti inflammatory drugs (Ibuprofen) just in case you have a gout attack. After you normalize, as long as you stay off sugar & alcohol you should remain gout free. Your uric acid may run high even after your body normalizes to the keto but the protective effects of BHB (beta hydroxybutyrate) may prevent a gout flare. You may want to talk to your doctor about a uric acid lowering drug. I would wait and see because after keto adaption some people see their UA drop into the normal range.

      I do appreciate your answer Really.
      Actually in my life a just got one attack triggered by covid prescription which was 8 months ago so far no attack. My problem is not about the gout but the long term devastating results of high uric acid in the body(dont know its true though lol)
      You have much experience..what is the average time period for getting adapted to see at least drop of uric acid or it may stay high also right?
      My glucose today was at the bottom level in blood tests..
      No drinking alcohol at all.
      I am just workingout so much shoud i reduce my weight lifting exercises too?

      @@Heisenberg991-m1i Keto adaption, on average, takes 2-3 months but in some people the process can take longer. It took about 1 year with me before my UA dropped from 9-10 to 8-8.5. At that point I went on allopurinol to drop my UA to 5-6. I think its pretty crucial to do a good job with the keto---no cheat days--because BHB inhibits the inflammation process and I haven't had any meaningful flares since starting keto.

      First month(exactly when ı stopped meds 300 allo a day ) 8.8 ua
      second month(on keto) 11.7
      third month(on keto) 9.8
      fourth month (on keto) 7.5
      fifth month (will be on keto) you guess ???
      will keep you updated..ı do modarate protein high fat ..
      may be better if i do high protein dont know yet..

      The data suggests that there is no problem being on keto with gout. I've been doing keto now for almost 2 years and staying at 50 grams of carbs per day. My uric acid averages between 7.5 to 8 mg/dL. I have not had any serious gout hits. Serious meaning debilitating. I did have some mild swelling once or twice but that was months ago. Keep the salt up and watch alcohol consumption.

      Being keto adapted seems to keep gout under control. So eating a low carb, high fat, moderate protein diet, if done correctly, will put you in a state of ketosis. If you are compliant with that way of eating and keep your ketones elevated then the BHB inhibits the inflammation process and you may see your gout attacks cease. That's the case for many of us out here that are ketogenic.

    I have gout issues and metabolic syndrome, possibly pre diabetic, etc. Take Chanca piedra for the attack, it also reduces inflammation. I feel the keto diet is dangerous, I feel Dr McDougall's diet is the best solution. I feel a low fat diet is the best way to go, reducing the fat intake solves the insulin resistance problem and with better control over glucose and insulin levels, the uric acid problem will go away. No facts to back up my hypothesis.

There is a body of science dating back into the 1800's that say otherwise but the bottom line is that all of us must choose an eating program that works for us on an individual level. As I tell any of the people that I work with I would rigorously monitor my bloods to track the progress of your eating plan. Its important to know if your markers are moving in the right direction.
