It is not a dumb question. It's a very important question. It was not adressed in the video because it was mnemonic video, not lecture. Coming to your question. Tumor size does not matter in T3 and T4 because in these stages, the tumor extends beyond the substance of organ of origin of the tumor. i.e. kidney, in this case. In other words, the tumor locally invades the surrouding tissue. When a tumor extends outside the original organ, it is almost always a poor prognostic factor and chances of patient's survival drastically decreases. So, if such invasion happens it is naturally a T3 or T4 stage, no matter how tiny the tumor may be, physically.
According to AJCC Cancer Staging Manual th Ed. Pg 743: In T3, Tumor extends into major veins or perinephric tissues. but not into the ipsilateral adrenal gland and not beyond Gerota’s fascia
