Thank you for this video. I am a guy and I am having pelvic floor dysfunction, luckily I’m seeing a PFPT and it’s helping. It’s one of the worst things I’ve ever experienced. Mine started with the combination of an incredible amount of stress and an anal Fissure. Luckily, the fissure was relatively small and it has healed. The last time I saw the colorectal doctor, he told me that the Fissure was 95% gone, and the pain I was having was pelvic floor dysfunction. I feel very lucky that my doctor knew enough to send me to a physical therapist. I also realize that I have suffered variations of pelvic floor dysfunction since I was a teenager. It is slowly getting better but it’s extremely uncomfortable. I have all of the symptoms you mentioned in this video. Thank you again for this video, it can feel very lonely. And it needn’t be. My physical therapist told me it is actually a very common issue and a lot of guys suffer from it. I definitely think there is also a huge stress component, the more I can stay calm and breathe, the less pain I have. Between the physical therapy and calming my nervous system, it’s getting better. But it’s a slow process! Thanks again.

@stdetain Yes, I have been regularly seeing a pelvic physio for two years now. She is great, and I feel much better following each appointment. Unfortunately, the relief is only temporary and my symptoms quickly return after a day or two. She has instructed me in the use of a pelvic wand but again, the relief is only temporary.
@@dwilliams3730 Remember that treating pelvic floor tension is only part of the local issues. Addressing posture, alignment, daily habits, and core strength can determine why the pelvic floor tension exists in the first place.

Somewhere the way: the muscles and vessels and nerves, from the testikel to the prostate, they are in there route disturbet, maybe by the pelvic area or prostate. And that gives problem in the testikel or groin or both.
So, as im writing this comment, im in the hospital, and it will be my 3rd day. I am suffering from intense pain in my left hip, which causes my infection + inflammation in pelvic muscle. How did i get this thing is a mystery that the doctors are finding and for the they did talking with me about my past like have i even fell, or had any injuries and there is no history and lastly they did biopsy, blood tests and mri... MRI and blood tests had shown signs of infection, and biopsy might tell more about my situation.... As of now, im on painkillers, and the doctors have started my treatment by giving me antibiotics... so yahh this my story

I had these symptoms 2 years ago and doctors gave me antibotics and said I had bacteria infection in my urine. I was on antibiotics for a few months then it went away for 4 months and I went back to working out. Then it came back and now I have a constant pressure in my bladder area under my belly button and sometimes when I eat certain foods I feel pain behind the butt area. Doctors now claim I have pelvic floor dysfunction. I also have bilateral varicose in the testicals. I don't even know where to start and how to get better
I had a ureteroscopy to remove a kidney stone and was left with this condition. One urologist told me I have none prostatitis chronic pelvic pain syndrome another urologist said I have tight muscles around the prostate. The pressure in the bladder is terrible! Also have pain in my scrotum my my anus my perineum my abdomen my lower back. Struggle to start urinating.
You should try to find a pelvic floor PT to help address the musculoskeletal pain and venous drainage/return in your testicles. I know how scary this must be for you.
I have a bad chronic muscle spasm up inside of my sphincter and have had everything done to me and im still in bad pain. It's been 2 years of hell and now im 22 kinda hopeless
This is a complex topic. There is CPPS, but there can be other causes such as compression of the pudendal nerve, etc. Physio works best if the problem truly is due to spasm of the pelvic floor musculature. Caution: if you have pelvic or buttock pain, stop ✋ exercise activities for starters- walking is benign, though. Avoid sitting. My husband grit his teeth and kept on biking, and is now in constant, chronic pain. He said it’s “not a headache in the pelvis, it’s burning and searing pain.” Mostly in perineum and unilateral. The rectal and penile pain persisted for the first year, but resolved. If you have similar issues, don’t try to tough it out. That worsens this type of issue. Biggest lesson we learned.

@@DefeatLust Go see a pelvic pain specialist, preferably a neurologist , and research pudendal neuralgia to see i it matches your symptoms. My husband was misdiagnosed several times. My husband is now 4 years in and has improved a lot with medication, avoiding biking and squatting, and doing research onthe condition. It’s known informally as Cyclist Syndrome as it was first described by a French doctor treating professional long distance cyclists. In neurology, it’s called Pudendal Neuralgia. How did you get injured??
@@recipehacker9752 Thank you for your reply and information. I drove cab 8-10 hours a day/6 days a week, with absolutely no physical exercise and/or walking for years. I started noticing penile and rectum pain and then it got MUCH worse after a few sexual encounters (no stds or anything like that). I'm quite sure mine is a strained pelvic floor, as it has improved a lot from me walking, deep breathing, and zero sexual activity. but I still urinate throughout the night and it is EXTREMELY frustrating...
Thank you for this explanation. I have had 2 surgeries in 6weeks for bladder cancer to remove a tumor then scar tissue. I have been sleeping in a recliner most nights and think it's possible that my pelvic muscles are causing my pain. Thanks for the info.

I've had pelvic floor therapy that didn't well because the female therapist seemed to be very uncomfortable dealing with my pelvis problem. I wondered why she went into the field with her dislike for the male pelvis.

Dear Nikhil, Your symptoms are common, yet distressing. The first thing we would advise is to go and see an MD to rule out an active infection. If you are cleared from any diagnosis that a doctor could treat, then you may be experiencing tension in your pelvic floor muscles. We hear this complaint of burning along the urethra with micturition (peeing) quite often. At 2.46 min A. you can see the muscles and vessels and nerves that are from the testikel to the prostate. And that is all the way your groin and that's what I think it is with me and only at my right side. B. Maybe it is not the pelvic muscles. Maybe it is just what I wrote at A. It is a problem after ejaculation, its like the strings are pulling there at A. The urologist check all, urine, prostate no increase, blood. He had nothing for me there.
Marlon, please keep in mind that not everyone with overactive bladder issues has a weak pelvic floor - some are tight! A pelvic exam by a pelvic health PT may help if strengthening the pelvic floor doesn't yield results.
Does this include testicular pain I want for every single test and they cannot find anything I am tapering off of Valium which affects the parasympathetic nerve any suggestions
Yes, this includes testicular pain as well. In lieu of Valium, you could try CBD oil capsules or suppositories made with CBD oil. Our patients are having good results with these alternatives, though there are not enough scientific studies performed that I can share with you.

I had pelvic pain since last 8 years. I think it begins when I fallen from tree with a sharp pain in the pelvic region before 8 years back. In the beginning there the symptoms are 1) Numbness 2) Stiffness 3) Tingling sensation 4) Constant burning pain 5) weakness of pelvis I consulted so many doctors but no improvement seen. Finally I went to neurologist who treated this as micro nerve injury of the pelvis. As above said some of symptoms minimised but weakness of pelvis is not improved. I consulted Physiotherapist who teaches keggel exercises but symptoms not disappeared. Symptoms increases following situations 1) lifting weights in farm work 2) In cold season 3) early morning hours 4) After ejaculaton / post orgasmic pain , weakness of pelvis Please, please, please help me this problem irritating , leading poor quality of life Madam I am waiting for your help.

Hi, I have symptoms,urine leakage specially in the night as I wake up in the morning there is urine leakage I see in my undies. I have gone through ultrasound,post boid residual, kidney, prostate and urine culture.no infections found. Taken medicine for chronic prostatitis but did not get relief. in the night I have to go for the pee around 5 times starting from night till waking time. Please help me what it could be since I have erectile dysfunction problem with these symptoms too.
What is your age , if it's above or around 50 then check your prostate , it may rise in size & create blockages on path of urine. Last month I had gone through same , but my B12 & Vitamin - D was low , now better , but under observation with medicines & supplements. You can also check for kidney performance status if it's ok.
You might have pelvic floor tension, as discussed in this video. Many of our clients are told they have prostatitis and are put on antibiotics...and then absolutely nothing changes. Urinary leakage, increased frequency and urgency are common with this condition. As is erectile dysfunction. Try to find a pelvic floor PT near you of you can. I hope this helps!

@@mateoslab As with any chronic pain condition, the spasm of the muscles get into a holding pattern. They become even more resistant to lengthening as time wears on and the pain/spasm cycle takes on a life of its own. The individual's brain also adapts to the pain and depression and anxiety typically increase in any painful condition when left untreated. I hope that answers your question.

I made a comment but basically the only pain I have is testicle pain after ejaculation, not mentioned in the video so idk If it applies or not. It happens maybe once or twice a month and lasts for no more than 5 minutes
@@mateoslab Testicular pain can be part of chronic pelvic pain syndrome, but in your case, the pain is intermittent and transient (and I am happy for you that it doesn't last too long). You might simply have some soft tissue restrictions in your testicles or the pain could be arising from your spine where the nerve that supplies the testicles exits the spinal column.
@@navinhemraz Sometimes testicular pain is the result of other components of the tissues around the perineum or abdominal wall. In other words, the pain in your testicle may not be arising from from the scrotal sack itself. What may help you is to find a pelvic floor PT who is comfortable assessing your abdominal wall, groin and the testicles to see if there are restrictions in the tissues which are driving your pain. It is possible that you had these restrictions and the pain was caused by this before and after the surgery, rather than the presence of a varicocele.
So today when I finished my football practice and after that the pain started in my rectum area for the time it was gone but at around 9 approx 4 hour later it started again after I peed I'm really afraid and searching about this everywear I don't know if I can play football again Please who so ever read this Please help me to get out of this pain it's very painful

How do you know if your muscles are too tight or too loose? I haven’t had much pain but I have a severe urge to urinate all the time. I’ve been to a urologist and my urine test is fine and he thinks I have overactive bladder.

Hello, I bave been suffering pelvic pain for years, it comes and goes. Bowel movements of course can get all the thing worse. For me, I use to strech those muscles befor a bowel movement, to fight this thing is neccesary to sleep more than 8 hours, calm stress, eat without sugar, bread, spicy or alcohol, and do exercise (never squats or andominals). Walk and not heavy gym can help a lot. I hope you get well soon. Eat a lot of vegetables. See a pelvic physioterapist, they can help a lot. I am not a medic but I have been long times without the pain, and when it comes, I do this whole thing to improve.
It is very difficult to discern the difference between pelvic floor tension and a bacterial infection of the prostate. Try going to a urologist first and then ask for a prescription for pelvic floor physical therapy. This way, you can cover all of your bases.

You might have something called proctalgia fugax. It is characterized by sudden and severe pain in the saddle region to the body which dissipates after 1-2 minutes. The cause of this condition is not clear, and may be the result of spasms of the external anal sphincter. One treatment remedy would be to attempt to perform deep abdominal breathing when the pain starts (though that is quite a challenge during severe pain episodes).

Your symptoms most probably point to tightened pelvic floor muscles. I can relate to this , as I've been in a similar condition before. You can try doing reverse kegels , there are plenty of info on the internet about how to do reverse kegels. Do those excercises not more than 2-3 times a day. And it may take some time depending on the severity of your condition. If it worked for me , I most probably think it would work for you as well.

I got Electroconvulsive Therapy after suffering from this brutal life-destroying disease for nearly two decades! Now my physical pain (extremely tight and bloated sensation throughout the lower abdomen) is largely relieved but my memory is very poor and I have reduced sense of ego and reduced motivation for doing anything. Before ECT life was like carrying a heavy burden I can never put down. These days I feel like a detached world-weary sleepwalking ghost!
Dear ZD Fang, I hear what you are saying. The medical community is all too quick to assign a psychological cause to male pelvic pain. The ECT may have been warranted for you, but it may not have been enough to address the physiological changes in your pelvis. You can still benefit from Pelvic Floor Physical Therapy. Don't let that ghost sensation make you feel as though there is nothing left for you to try. Abdominal bloating and distention is something that can be treated by a PT. I hope that you can find someone who can address the problems that continue to bother you. Chronic Male Pelvic Pain Syndrome is not a psychological disorder. It is a physical one. Thanks for your response. I had actually tried physical therapy before eventually resorting to ECT. I used a specially designed self-help wand to do internal therapy and it seemed to be quite effective in releasing the tender trigger points on the pelvic floor, which reduced my urinary symptoms. But the abdominal discomfort was largely unchanged, perhaps due to my case being too severe and deeply entrenched. By inducing whole body neuromuscular convulsion, ECT therapy had managed to break loose some tight and knotted muscles in my lower abdomen. During last three years after taking ECT the abdominal bloating has gradually reduced to a discomfort level of 1-2 points on a scale of 0-10 points (before ECT it's about 5-6). The ghost/apathetic feeling was quite extreme immediately following ECT although has now recovered substantially. ECT was a crude and violent and inherently risky medical procedure. If one's livelihood and life satisfaction depends on having a sharp intellect/memory (like for example a professional programmer or writer), I would definitely not recommend it. I have a pain behind my rectum but it's not my tailbone. It's just below the tailbone and behind my rectum. Any idea what that might be? I've had prostate infections in the past. Relaxation of the pelvic floor muscles should never cause priapism. This is an urgent medical matter which needs prompt medical attention to get the blood out of the penis in order to prevent necrosis or tissue death within the penile tissues.
MY PERINEAL TRAUMA - i had a trauma on my perineum 10 years ago when i jumped up in my room to catch a ball & landed my perineum on the edge of a bed. i was moaning in pain for 30 min or so but it went away & I experienced no discomfort for 10 years since that happened. since an year ago I started riding bike everyday on an uneven terrain to go to work everyday & the pain from 10 years ago resurfaced like a ghost as my perineum kept pressing against the seat. once in a week i have this episode where I pee 5 times in an hour. also if dont drink enough water, my urethra burns like hell when peeing. if i drink enough water then there is no problem at all. when i press my hand against my perineum i can feel the pain. so far no erectile dysfunction even though i masturbate like 3-4 times a week. i hope it doesn't get serious from here on. i also do a desk job & am overweight too. but i still cant believe how a trauma from 10 years ago could do this to me now.

As fas as I know , cycling is one of the worst ways to injure your perenium. So does riding motorcycle. Maybe that's what has caused you to again feel the pain in your perenium. Activities like riding a motorcycle , a bicycle are very unhealthy for the perenium.
I had a skateboarding accident when I was a kid. I pogo sticked it and I really never had symptoms until later on that went away and now the last year it’s been a battle. I’m starting to do inner work and that works great. Suck it up get a wand or anything. Those internal muscles and nerves need to be massaged
I have the heavy pressure symptom in the rectum and chronic constipation. It is very difficult to pass gas or stool when sitting. It is possible that I have Hirschsprung's disease or my intestines have collapsed into my pelvic area. There is also an odour emanating from me constantly. Doctors have failed me. Dear Xierxu, Your symptoms are indeed troubling, but they are more common than you might think. Firstly, a gastroenterologist would be helpful, and he or she may suggest a test called manometry. This involves the insertion of a balloon into the rectum, so that your physician can see the actions of your bowel wall and sphincter muscles which must coordinate together for regular bowel movements to occur. After that, you can seek out the advice of a pelvic floor PT. We are well-trained in the mechanics of defecation; in lay terms, we know what positions, relaxation techniques and breathing patterns to aid in effectively treating chronic constipation. A GI doctor can help you to rule out some of the diagnoses you have listed. A pelvic floor PT can help you learn to release gas and stool in the least embarrassing and most easeful manner possible. Getting in touch with your pelvic floor muscles will take some time. Try inhaling and letting the air expand into your belly. As you practice this, imagine air going down into your groin. Do you know how it feels when you sit on a toilet and your groin muscles balloon out into the bowl of the toilet? This is the same premise. Try to find a pelvic floor physical therapist so you can learn how to do this. You are holding tension in your groin muscles in the same way that a person who has a neck strain holds their tension by hunching their shoulders.

@@beccaironside1148 I talked to my urologist about cpps but he says it happens only to women and there is no Physical therapist specialized in pelvic floor. I did try stretching it stopped my frequent urination. The only access that I have is chiropractor. Americans and Canadians are lucky because people understand problem and have physical therapy program for that.
I'm still suffering this condition. Is it normal to be in really bad pain after a bowel movement? I feel like I'm losing my mind because the doctors just are not helping! I saw a different doctor on Friday and he checked my prostate and said it was fine no abnormalities. I'm absolutely lost! Is it normal to have pain in my abdomen bladder anus and groin with this ? I have the same thing except I also have prostate trouble. I just had a biopsy and will find out the results in a few days. They also put a scope up my urethra into my bladder. I was sedated when they did it of course...but when I woke up...EXTREME PAIN! I'm going to a physical therapist right now but it's only been a few days so no relief yet. @@jack_knife-1478 Yeah, I have that plus my penis was on fire. My Urologist sent me to a physical therapist and the assorted excercises she's having me do is working. It all has to do with a weak pelvic floor. I thought it was a load of BS but I'm starting to feel better.
To everyone who has responded to this inquiry, I am sorry it has taken me this long to reply! Pain after a bowel movements can be driven my tight pelvic floor muscles. The same can be attributed to urinary pain. Most people clench their pelvic floor muscles during bowel movements. This is the opposite of what the body intended. If the pelvic floor muscles are tight already and a person contracts them while pooping, then the entire groin goes into a tailspin, and pain and spasms are the inevitable result. There is treatment for urinary pain and rectal spasms in pelvic floor PT, provided the individual has ruled out other causes in the medical community, which sounds like the case here. The treatment from a PT standpoint would be to relax the pelvic floor muscles and the abdomen as well, which plays a significant role during urination and BM's. At last we have a forum for the most intimate and embarrassing symptoms possible. So proud of everyone who has submitted questions here!

I am so sorry to hear about what you are going through. Any surgery to the lumbar spine can cause pelvic floor tension, as the lumbar spine muscles attach to the pelvis. I truly believe that pelvic floor therapy could help with your testicular pain during and after sex. If you cannot find a pelvic floor PT near you, try finding a PT for your lumbar spine and see if treatment to this area will decrease your pain with coitus.

Couple of months ago I added stomach vacuums to my strength training program. After one month of doing the vacuums I decided to stop them...... and then I gradually started having ED and some dull penile pain. My blood and urine test were ok, and my urologist said I have non bacterial chronic prostatitis and didn't mention where the ED and pain are coming from. Recently I understood that the stomach vacuums are a pelvic floor exercise similar to kegels. Do you think my problems are due to those pelvic floor exercises? I suspect a correlation but how can I tell if my pelvic floor muscles are in spasm? In my country there are no physiotherapists specialized in pelvic floor.
Yes, it makes sense that the vacuum stomach exercises would cause increased tension in your pelvic floor muscles, Lyudmil. My suspicion is that you have some pelvic floor tension, but that is difficult for patients to determine themselves. Well I had the same after a training for 2 months losing 26 lbs, thought I had prostatitis, got tested everything was -ve, the reverse kegels were recommended, tried it and on the first day, 90% of the pain was gone 💪💪💪

Sneezing forces a tremendous amount of downward pressure into the pelvic musculature. This can cause pelvic pain or urinary leakage with sneezing. These symptoms may persist at other times as well.

After about 3 months of research and ruling out std, cancer and uti, i think it may be a muscle spasm of one of the pelvic floor muscle. The sensation I feel is in my scrotum around the testicles. It’s a weird tingling crawling sensation. After about 2 months of feeling this I noticed my testicles actually move when the sensation occurs. I’m thinking it’s the cremaster muscle, the one that surrounds the testies and raises them when temperatures are cold and lowers them when temperatures are warm. Try doing reverse kegels exercise. You most likely have an overly tight pelvic floor. But remember to not overdo it. Or else you'd end up with a weakened pelvic floor. 3 times a day should be fine.

Anyone here get perineum pain when landing from a jump? or like landing from a 2 feet or 3 feet of air. It's a sharp pain, but it goes away within like couple of seconds. I had this problem since I was in middle school many years ago. I got kicked in the perineum area by a kid at my school and I think that what caused me to start having this pain. For real I’ve had this issue since the same time I’m 31 now but when I land from said jump doesn’t have to be very high it last for couple of minutes if not longer and I’m running around trying to walk it off…. Did you ever find out anything to help or what it is ???

Inner thigh pain is consistent with the pathways of the nerves which supply the pelvic floor, and is often seen with this diagnosis. Try gently stretching your inner thighs to see if you can get some relief. There is video on this UA-cam channel entitled Self treatment for Pelvic Pain. There are some soft tissue glides you can also do to help ease this muscular tension.

@@95bloc97 I would make sure to eat foods that make you go or take miralax ( if you’re not already doing that). There are many exercises out there that help . For me, staying active, applying heat and sleeping is when it didn’t bother me. Driving in a car, sitting, not moving around made it worse. You may want to look into a possible cause being a bulging disk or disks in your back that may be compressing Mercer’s that run through the pelvic floor. An MRI would show that and then maybe you could get that script?You may want to get in touch with a PT who specializes in pelvic floor therapy and explain you’re having a hard time getting a doctor to write a script. They may be able to help with suggestions on how to relive or cope with the problem. I have flare ups when I have chocolate, citrus drinks or fruits, caffeine . They are all triggers. Too much of it and I feel it. Alcohol is another one that is a trigger although I don’t drink so that’s not an issue. The same things that trigger bladder issues seem to bother my pelvic floor.

@Tony Wijaya Dear Tony, Pudental Neuralgia is a type of chronic male pelvic pain syndrome. PN is an irritation of the branch of the sciatic nerve. This pudental nerve supplies sensation to the pelvic floor muscles. Pudental Neuralgia symptoms involve shock-like, burning pain into the groin. It is a specific diagnosis, whereas chronic male pelvic pain syndrome is characterized by pain that is a little less electric and searing, but is still incredibly problematic as described in this video. That said, physical therapy is helpful for pudental neuralgia. We treat it quite a bit. It might also help to see a pain management physician who specializes in pudental neuralgia. I hope this helps. This condition has changed my life it's horrific! And it all started after having a operation to have a kidney stone removed from my ureter. My scrotum hurts my bladder hurts my penis hurts my perenium hurts and my anus. I've had this for 8 months now and don't even want to wake up in the morning!! My quality of life is horrible because of this and its actually making me suicidal!!! My urologist said I have chronic pelvic pain and unfortunately they can't do anything about it. They tried me with antibiotics but didn't work. And I can't think why a male wouldn't seek medical attention for this because it's a horrible debilitating condition!!!!!
Thank you for writing this for fellow sufferers. This is a very challenging condition, and many men are given antibiotics that don't really address what is going on. I hope that you can find a pelvic floor physical therapist or acupuncturist who can treat this in a different way. In the meantime, we know what you are going through and it is incredibly difficult.

MY PERINEAL TRAUMA - i had a trauma on my perineum 10 years ago when i jumped up in my room to catch a ball & landed my perineum on the edge of a bed. i was moaning in pain for 30 min or so but it went away & I experienced no discomfort for 10 years since that happened. since an year ago I started riding bike everyday on an uneven terrain to go to work everyday & the pain from 10 years ago resurfaced like a ghost as my perineum kept pressing against the seat. once in a week i have this episode where I pee 5 times in an hour. also if dont drink enough water, my urethra burns like hell when peeing. if i drink enough water then there is no problem at all. when i press my hand against my perineum i can feel the pain. so far no erectile dysfunction even though i masturbate like 3-4 times a week. i hope it doesn't get serious from here on. i also do a desk job & am overweight too. but i still cant believe how a trauma from 10 years ago could do this to me now.
I don't have sharp pelvic pain but i do feel aching and tightness in the pelvic region i believe the resting level of my pelvic floor is too high is it possible to have erectile dysfunction without pelvic pain present? I have the urinary and erectile issues just not any sharp pelvic pain.
Dear Solomon, The aching and tightness in your pelvic region may be driven by tight muscles. Urinary hesitancy and erectile dysfunction are indicators of this muscle tension. A quick tip for you may be to sit down while urinating. This will allow the muscles of your pelvic floor to stretch open and balloon downwards, thereby easing the flow of the urine stream. For the erectile dysfunction, you can seek out pelvic floor physical therapy. This is treatable and so very common! Thanks for this message you wrote. Sitting down while urinating is very helpful to men, especially if they have symptoms of pain in the pelvis, or the sensation of urinary hesitation, dribbling or incomplete voiding. If you are sitting while urinating, try not to "push" the urine out. Instead, try to imagine that your perineum is ballooning out into the toilet bowl. Do some deep breathing in this position and you should feel your pelvic floor relax. The urine should just come out without pushing. The same goes for bowel movements. Becca Ironside, PT
I had an operation to remove a kidney stone and ever since I've had this. My urologist did an examination of my prostate and said it was fine but the muscles around it are really tight (hypo sensitive) is this my pelvic floor? Dear Naig, Yes, people with chronic male pain syndrome find riding a bike challenging, because the seat of the bike compresses the pelvic floor muscles. You can try looking for a specialty bike seat to take pressure off of this area (they sell them!). Alternatively, you could find another way to exercise or another form of transport. Greetings, Humberto. Some men with pelvic floor muscle tension do report frequent urination. One suggestion we would make for them would be to sit down on the toilet seat while peeing. This serves the purpose of allowing the pelvic floor muscles to relax and thereby emptying the bladder more fully. We have seen this work in practice to decrease urinary frequency in men.

Thank you for this video. I am a guy and I am having pelvic floor dysfunction, luckily I’m seeing a PFPT and it’s helping. It’s one of the worst things I’ve ever experienced. Mine started with the combination of an incredible amount of stress and an anal Fissure. Luckily, the fissure was relatively small and it has healed. The last time I saw the colorectal doctor, he told me that the Fissure was 95% gone, and the pain I was having was pelvic floor dysfunction. I feel very lucky that my doctor knew enough to send me to a physical therapist. I also realize that I have suffered variations of pelvic floor dysfunction since I was a teenager. It is slowly getting better but it’s extremely uncomfortable. I have all of the symptoms you mentioned in this video. Thank you again for this video, it can feel very lonely. And it needn’t be. My physical therapist told me it is actually a very common issue and a lot of guys suffer from it. I definitely think there is also a huge stress component, the more I can stay calm and breathe, the less pain I have. Between the physical therapy and calming my nervous system, it’s getting better. But it’s a slow process! Thanks again.

@stdetain Yes, I have been regularly seeing a pelvic physio for two years now. She is great, and I feel much better following each appointment. Unfortunately, the relief is only temporary and my symptoms quickly return after a day or two. She has instructed me in the use of a pelvic wand but again, the relief is only temporary.

I had these symptoms 2 years ago and doctors gave me antibotics and said I had bacteria infection in my urine. I was on antibiotics for a few months then it went away for 4 months and I went back to working out. One urologist told me I have none prostatitis chronic pelvic pain syndrome another urologist said I have tight muscles around the prostate. The pressure in the bladder is terrible! You should try to find a pelvic floor PT to help address the musculoskeletal pain and venous drainage/return in your testicles. I know how scary this must be for you.

This is a complex topic. There is CPPS, but there can be other causes such as compression of the pudendal nerve, etc. Physio works best if the problem truly is due to spasm of the pelvic floor musculature. Caution: if you have pelvic or buttock pain, stop ✋ exercise activities for starters- walking is benign, though. Avoid sitting. My husband grit his teeth and kept on biking, and is now in constant, chronic pain. He said it’s “not a headache in the pelvis, it’s burning and searing pain.” Mostly in perineum and unilateral. The rectal and penile pain persisted for the first year, but resolved. If you have similar issues, don’t try to tough it out. That worsens this type of issue. Biggest lesson we learned.

My husband is now 4 years in and has improved a lot with medication, avoiding biking and squatting, and doing research onthe condition. It’s known informally as Cyclist Syndrome as it was first described by a French doctor treating professional long distance cyclists. In neurology, it’s called Pudendal Neuralgia. How did you get injured??

I drove cab 8-10 hours a day/6 days a week, with absolutely no physical exercise and/or walking for years. I started noticing penile and rectum pain and then it got MUCH worse after a few sexual encounters (no stds or anything like that).
I'm quite sure mine is a strained pelvic floor, as it has improved a lot from me walking, deep breathing, and zero sexual activity. but I still urinate throughout the night and it is EXTREMELY frustrating...

Dear Nikhil, Your symptoms are common, yet distressing. The first thing we would advise is to go and see an MD to rule out an active infection. If you are cleared from any diagnosis that a doctor could treat, then you may be experiencing tension in your pelvic floor muscles.
We hear this complaint of burning along the urethra with micturition (peeing) quite often. This symptom is often consistent with having pelvic floor tightness and could be helped by finding a pelvic floor physical therapist.

Marlon, please keep in mind that not everyone with overactive bladder issues has a weak pelvic floor - some are tight! A pelvic exam by a pelvic health PT may help if strengthening the pelvic floor doesn't yield results.

Yes, this includes testicular pain as well. In lieu of Valium, you could try CBD oil capsules or suppositories made with CBD oil. Our patients are having good results with these alternatives, though there are not enough scientific studies performed that I can share with you.

Has your pain subsided yet? You are correct that performing pelvic floor exercises can exacerbate this condition if performed too aggressively or in conjunction with heavy lifting or squats at the gym.

I had pelvic pain since last 8
years. I think it begins when I fallen from tree with a sharp pain in the pelvic region before 8 years back. In the beginning there the symptoms are
1) Numbness 2) Stiffness
3) Tingling sensation
4) Constant burning pain
5) weakness of pelvis
I consulted so many doctors but no improvement seen. Finally I went to neurologist who treated this as micro nerve injury of the pelvis. As above said some of symptoms minimised but weakness of pelvis is not improved. I consulted Physiotherapist who teaches keggel exercises but symptoms not disappeared.
Symptoms increases following situations
1) lifting weights in farm work
2) In cold season
3) early morning hours
4) After ejaculaton / post orgasmic pain , weakness of pelvis

Hi, I have symptoms,urine leakage specially in the night as I wake up in the morning there is urine leakage I see in my undies. I have gone through ultrasound,post boid residual, kidney, prostate and urine culture.no infections found. Taken medicine for chronic prostatitis but did not get relief. in the night I have to go for the pee around 5 times starting from night till waking time. Please help me what it could be since I have erectile dysfunction problem with these symptoms too.
What is your age , if it's above or around 50 then check your prostate , it may rise in size & create blockages on path of urine. Last month I had gone through same , but my B12 & Vitamin - D was low , now better , but under observation with medicines & supplements. You can also check for kidney performance status if it's ok.
You might have pelvic floor tension, as discussed in this video. Many of our clients are told they have prostatitis and are put on antibiotics...and then absolutely nothing changes. Urinary leakage, increased frequency and urgency are common with this condition. As is erectile dysfunction. Try to find a pelvic floor PT near you of you can. I hope this helps!

@@mateoslab As with any chronic pain condition, the spasm of the muscles get into a holding pattern. They become even more resistant to lengthening as time wears on and the pain/spasm cycle takes on a life of its own. The individual's brain also adapts to the pain and depression and anxiety typically increase in any painful condition when left untreated. I hope that answers your question.

I made a comment but basically the only pain I have is testicle pain after ejaculation, not mentioned in the video so idk If it applies or not. It happens maybe once or twice a month and lasts for no more than 5 minutes
@@mateoslab Testicular pain can be part of chronic pelvic pain syndrome, but in your case, the pain is intermittent and transient (and I am happy for you that it doesn't last too long). You might simply have some soft tissue restrictions in your testicles or the pain could be arising from your spine where the nerve that supplies the testicles exits the spinal column.

@@navinhemraz Sometimes testicular pain is the result of other components of the tissues around the perineum or abdominal wall. In other words, the pain in your testicle may not be arising from from the scrotal sack itself. What may help you is to find a pelvic floor PT who is comfortable assessing your abdominal wall, groin and the testicles to see if there are restrictions in the tissues which are driving your pain. It is possible that you had these restrictions and the pain was caused by this before and after the surgery, rather than the presence of a varicocele.

Have you tried antibiotics? Sounds like prostititis. you will need to take antibiotics for an extended period of time 2 weeks or 1 month is not enough

Hello, I bave been suffering pelvic pain for years, it comes and goes. Bowel movements of course can get all the thing worse. For me, I use to strech those muscles befor a bowel movement, to fight this thing is neccesary to sleep more than 8 hours, calm stress, eat without sugar, bread, spicy or alcohol, and do exercise (never squats or andominals). Walk and not heavy gym can help a lot. I hope you get well soon. Eat a lot of vegetables. See a pelvic physioterapist, they can help a lot. I am not a medic but I have been long times without the pain, and when it comes, I do this whole thing to improve.
How do I know if it is Pelvis or Prostate?
It is very difficult to discern the difference between pelvic floor tension and a bacterial infection of the prostate. Try going to a urologist first and then ask for a prescription for pelvic floor physical therapy. This way, you can cover all of your bases.

You might have something called proctalgia fugax. It is characterized by sudden and severe pain in the saddle region to the body which dissipates after 1-2 minutes. The cause of this condition is not clear, and may be the result of spasms of the external anal sphincter. One treatment remedy would be to attempt to perform deep abdominal breathing when the pain starts (though that is quite a challenge during severe pain episodes).

Your symptoms most probably point to tightened pelvic floor muscles. I can relate to this , as I've been in a similar condition before. You can try doing reverse kegels , there are plenty of info on the internet about how to do reverse kegels. Do those excercises not more than 2-3 times a day. And it may take some time depending on the severity of your condition. If it worked for me , I most probably think it would work for you as well.

I got Electroconvulsive Therapy after suffering from this brutal life-destroying disease for nearly two decades! Now my physical pain (extremely tight and bloated sensation throughout the lower abdomen) is largely relieved but my memory is very poor and I have reduced sense of ego and reduced motivation for doing anything. Before ECT life was like carrying a heavy burden I can never put down. These days I feel like a detached world-weary sleepwalking ghost!
Dear ZD Fang, I hear what you are saying. The medical community is all too quick to assign a psychological cause to male pelvic pain. The ECT may have been warranted for you, but it may not have been enough to address the physiological changes in your pelvis. You can still benefit from Pelvic Floor Physical Therapy. Don't let that ghost sensation make you feel as though there is nothing left for you to try. Abdominal bloating and distention is something that can be treated by a PT. I hope that you can find someone who can address the problems that continue to bother you.

I had actually tried physical therapy before eventually resorting to ECT. I used a specially designed self-help wand to do internal therapy and it seemed to be quite effective in releasing the tender trigger points on the pelvic floor, which reduced my urinary symptoms. But the abdominal discomfort was largely unchanged, perhaps due to my case being too severe and deeply entrenched.
By inducing whole body neuromuscular convulsion, ECT therapy had managed to break loose some tight and knotted muscles in my lower abdomen. During last three years after taking ECT the abdominal bloating has gradually reduced to a discomfort level of 1-2 points on a scale of 0-10 points (before ECT it's about 5-6). The ghost/apathetic feeling was quite extreme immediately following ECT although has now recovered substantially.
ECT was a crude and violent and inherently risky medical procedure. If one's livelihood and life satisfaction depends on having a sharp intellect/memory (like for example a professional programmer or writer), I would definitely not recommend it.

Relaxation of the pelvic floor muscles should never cause priapism. This is an urgent medical matter which needs prompt medical attention to get the blood out of the penis in order to prevent necrosis or tissue death within the penile tissues.
MY PERINEAL TRAUMA -
i had a trauma on my perineum 10 years ago when i jumped up in my room to catch a ball & landed my perineum on the edge of a bed. i was moaning in pain for 30 min or so but it went away & I experienced no discomfort for 10 years since that happened.
since an year ago I started riding bike everyday on an uneven terrain to go to work everyday & the pain from 10 years ago resurfaced like a ghost as my perineum kept pressing against the seat. once in a week i have this episode where I pee 5 times in an hour. also if dont drink enough water, my urethra burns like hell when peeing. if i drink enough water then there is no problem at all.
when i press my hand against my perineum i can feel the pain. so far no erectile dysfunction even though i masturbate like 3-4 times a week.
i hope it doesn't get serious from here on. i also do a desk job & am overweight too.
but i still cant believe how a trauma from 10 years ago could do this to me now.

As fas as I know , cycling is one of the worst ways to injure your perenium. So does riding motorcycle. Maybe that's what has caused you to again feel the pain in your perenium.
Activities like riding a motorcycle , a bicycle are very unhealthy for the perenium.
I had a skateboarding accident when I was a kid. I pogo sticked it and I really never had symptoms until later on that went away and now the last year it’s been a battle. I’m starting to do inner work and that works great. Suck it up get a wand or anything. Those internal muscles and nerves need to be massaged

Your symptoms are indeed troubling, but they are more common than you might think. Firstly, a gastroenterologist would be helpful, and he or she may suggest a test called manometry. This involves the insertion of a balloon into the rectum, so that your physician can see the actions of your bowel wall and sphincter muscles which must coordinate together for regular bowel movements to occur.
After that, you can seek out the advice of a pelvic floor PT. We are well-trained in the mechanics of defecation; in lay terms, we know what positions, relaxation techniques and breathing patterns to aid in effectively treating chronic constipation.
A GI doctor can help you to rule out some of the diagnoses you have listed. A pelvic floor PT can help you learn to release gas and stool in the least embarrassing and most easeful manner possible.

How do you stop pelvic floor spasms and how can you keep it relaxed
Getting in touch with your pelvic floor muscles will take some time. Try inhaling and letting the air expand into your belly. As you practice this, imagine air going down into your groin.
Do you know how it feels when you sit on a toilet and your groin muscles balloon out into the bowl of the toilet? This is the same premise.
Try to find a pelvic floor physical therapist so you can learn how to do this. You are holding tension in your groin muscles in the same way that a person who has a neck strain holds their tension by hunching their shoulders.

I'm still suffering this condition. Is it normal to be in really bad pain after a bowel movement? I feel like I'm losing my mind because the doctors just are not helping! I have the same thing except I also have prostate trouble. I just had a biopsy and will find out the results in a few days. They also put a scope up my urethra into my bladder. I was sedated when they did it of course...but when I woke up...EXTREME PAIN! I'm going to a physical therapist right now but it's only been a few days so no relief yet. My whole pelvic area is in pain. I can barely walk. I'm ready to trade my body in for a new model. :/

To everyone who has responded to this inquiry, I am sorry it has taken me this long to reply! Pain after a bowel movements can be driven my tight pelvic floor muscles. The same can be attributed to urinary pain.
Most people clench their pelvic floor muscles during bowel movements. This is the opposite of what the body intended. If the pelvic floor muscles are tight already and a person contracts them while pooping, then the entire groin goes into a tailspin, and pain and spasms are the inevitable result.
There is treatment for urinary pain and rectal spasms in pelvic floor PT, provided the individual has ruled out other causes in the medical community, which sounds like the case here. The treatment from a PT standpoint would be to relax the pelvic floor muscles and the abdomen as well, which plays a significant role during urination and BM's.

I am so sorry to hear about what you are going through. Any surgery to the lumbar spine can cause pelvic floor tension, as the lumbar spine muscles attach to the pelvis. I truly believe that pelvic floor therapy could help with your testicular pain during and after sex. If you cannot find a pelvic floor PT near you, try finding a PT for your lumbar spine and see if treatment to this area will decrease your pain with coitus.

Could be a yeast infection if it emits a foul odour/smell. He should see a doctor.
Couple of months ago I added stomach vacuums to my strength training program. After one month of doing the vacuums I decided to stop them...... and then I gradually started having ED and some dull penile pain. My blood and urine test were ok, and my urologist said I have non bacterial chronic prostatitis and didn't mention where the ED and pain are coming from. Recently I understood that the stomach vacuums are a pelvic floor exercise similar to kegels. Do you think my problems are due to those pelvic floor exercises? I suspect a correlation but how can I tell if my pelvic floor muscles are in spasm? In my country there are no physiotherapists specialized in pelvic floor.
Yes, it makes sense that the vacuum stomach exercises would cause increased tension in your pelvic floor muscles, Lyudmil. My suspicion is that you have some pelvic floor tension, but that is difficult for patients to determine themselves. Well I had the same after a training for 2 months losing 26 lbs, thought I had prostatitis, got tested everything was -ve, the reverse kegels were recommended, tried it and on the first day, 90% of the pain was gone 💪💪💪
Thanks for mentioning the ED. I've had so-called prostatitis for over 20 years, but only recently started having right testicular pain and ED. Freaked me right out. Glad to know I'm not alone. Stay positive, guys!

Sneezing forces a tremendous amount of downward pressure into the pelvic musculature. This can cause pelvic pain or urinary leakage with sneezing. These symptoms may persist at other times as well.

After about 3 months of research and ruling out std, cancer and uti, i think it may be a muscle spasm of one of the pelvic floor muscle. The sensation I feel is in my scrotum around the testicles. It’s a weird tingling crawling sensation. After about 2 months of feeling this I noticed my testicles actually move when the sensation occurs. I’m thinking it’s the cremaster muscle, the one that surrounds the testies and raises them when temperatures are cold and lowers them when temperatures are warm. Now, if I could only get it to relax :(

You might have tension in your pelvic floor muscles causing these symptoms. Have you seen any specialists about this?

Probably you had a penile fracture, I'd recommend that you visit a urologist or an ER close to you, wish you quick recovery.
Pelvic pain has ruined my life, I feel like ending it all sometimes. I'm only 28 years old, stabbing pains in my perineum that no doctor could find anything wrong with me. I can't sit down, can't workout and the pain doesn't go away.
Have you tried Balcofen (muscle relaxer)?

Try doing reverse kegels exercise. You most likely have an overly tight pelvic floor. But remember to not overdo it. Or else you'd end up with a weakened pelvic floor. 3 times a day should be fine.
@@madhisathya2766 How do I know if my pelvic floor is tight or weakened? For example when I'm urgent to pee, I hold back my urine only to get stabbing pains. The same pain also comes when I cough or sneeze. The pain is so bad I drop to the floor sometimes , and I used to do Martial arts so I can take physical pain but not this. It's too much

Anyone here get perineum pain when landing from a jump? or like landing from a 2 feet or 3 feet of air. It's a sharp pain, but it goes away within like couple of seconds. I had this problem since I was in middle school many years ago. I got kicked in the perineum area by a kid at my school and I think that what caused me to start having this pain. I'm too embarrassed to tell anyone :/
For real I’ve had this issue since the same time I’m 31 now but when I land from said jump doesn’t have to be very high it last for couple of minutes if not longer and I’m running around trying to walk it off…. Did you ever find out anything to help or what it is ???
Same, af. I always thought it came from lifting heavy weights. Again was a teen when this came about. Sometimes comes out of no where when just sitting. Not often, but sometimes.

Inner thigh pain is consistent with the pathways of the nerves which supply the pelvic floor, and is often seen with this diagnosis. Try gently stretching your inner thighs to see if you can get some relief. There is video on this UA-cam channel entitled Self treatment for Pelvic Pain. There are some soft tissue glides you can also do to help ease this muscular tension.

@@95bloc97 I would make sure to eat foods that make you go or take miralax ( if you’re not already doing that). There are many exercises out there that help . For me, staying active, applying heat and sleeping is when it didn’t bother me. Driving in a car, sitting, not moving around made it worse. You may want to look into a possible cause being a bulging disk or disks in your back that may be compressing Mercer’s that run through the pelvic floor. An MRI would show that and then maybe you could get that script?You may want to get in touch with a PT who specializes in pelvic floor therapy and explain you’re having a hard time getting a doctor to write a script. They may be able to help with suggestions on how to relive or cope with the problem. I have flare ups when I have chocolate, citrus drinks or fruits, caffeine . They are all triggers. Too much of it and I feel it. Alcohol is another one that is a trigger although I don’t drink so that’s not an issue. The same things that trigger bladder issues seem to bother my pelvic floor.

Its no prostate increase.
No bacterial.
And its always on the right side, testicle right and right inguinal area.
Many men have no discernable prostate problems nor bacteria to pin these symptoms upon. Have you found help for your condition?

The year 2019 i had a checkup.
Prostate ultrasound check.
Blood check.
Urine check.
All that was good.
So all cost 500 euro in a private hosp.
So far no result.
The eurologist docters know to little of this problem.

No not yet, because I have insurance 9 times a year a threatment by a manuel theraphy for my backpain.
The pain is just like muscle strain in the pelvic region.
Are there special exercise?
I'm experiencing sudden loss of flacid penis size and it is worst when I am pooping. And while pooping the anus feels comparatively way looser than before. What could be the reason ? I don't have pooping issues as of now but still am concerned.
Does this include scrotum pain , testicle pain ?
Dear Darren, The answer to your question is yes. Many of our clients with tight pelvic floor muscles experience testicular and scrotal pain. The pain is often made worse with sex, wearing tight clothing or prolonged sitting.

Ok that makes sense cause when I massage my pelvic floor muscle s with hands the pain will disappear or travel to another location, it’s pretty crazy how fast it changes, I’ve had this for 10 years comes in go , I’ve been able to perform my own plan but if shit gets worse I’ll come see you , I definitely don’t do Kegals cause that doesn’t help . @Tony Wijaya Dear Tony, Pudental Neuralgia is a type of chronic male pelvic pain syndrome. PN is an irritation of the branch of the sciatic nerve. This pudental nerve supplies sensation to the pelvic floor muscles. Pudental Neuralgia symptoms involve shock-like, burning pain into the groin. It is a specific diagnosis, whereas chronic male pelvic pain syndrome is characterized by pain that is a little less electric and searing, but is still incredibly problematic as described in this video. That said, physical therapy is helpful for pudental neuralgia. We treat it quite a bit. It might also help to see a pain management physician who specializes in pudental neuralgia. I hope this helps.

This condition has changed my life it's horrific! And it all started after having a operation to have a kidney stone removed from my ureter. My scrotum hurts my bladder hurts my penis hurts my perenium hurts and my anus. I've had this for 8 months now and don't even want to wake up in the morning!! My quality of life is horrible because of this and its actually making me suicidal!!! My urologist said I have chronic pelvic pain and unfortunately they can't do anything about it. They tried me with antibiotics but didn't work. And I can't think why a male wouldn't seek medical attention for this because it's a horrible debilitating condition!!!!!
Thank you for writing this for fellow sufferers. This is a very challenging condition, and many men are given antibiotics that don't really address what is going on. I hope that you can find a pelvic floor physical therapist or acupuncturist who can treat this in a different way. In the meantime, we know what you are going through and it is incredibly difficult.

@@beccaironside1148 just been to doctors today and they're sending me back to urologist. The doctor I saw today thinks I have hypotonic pelvic floor. All I have been getting told is I have chronic pelvic pain syndrome prostatitis and now hypotonic pelvic floor? Im exhausted with it all.
MY PERINEAL TRAUMA -
i had a trauma on my perineum 10 years ago when i jumped up in my room to catch a ball & landed my perineum on the edge of a bed. i was moaning in pain for 30 min or so but it went away & I experienced no discomfort for 10 years since that happened.
since an year ago I started riding bike everyday on an uneven terrain to go to work everyday & the pain from 10 years ago resurfaced like a ghost as my perineum kept pressing against the seat. once in a week i have this episode where I pee 5 times in an hour. also if dont drink enough water, my urethra burns like hell when peeing. if i drink enough water then there is no problem at all.
when i press my hand against my perineum i can feel the pain. so far no erectile dysfunction even though i masturbate like 3-4 times a week.
i hope it doesn't get serious from here on. i also do a desk job & am overweight too.
but i still cant believe how a trauma from 10 years ago could do this to me now.
I don't have sharp pelvic pain but i do feel aching and tightness in the pelvic region i believe the resting level of my pelvic floor is too high is it possible to have erectile dysfunction without pelvic pain present? I have the urinary and erectile issues just not any sharp pelvic pain.
Dear Solomon, The aching and tightness in your pelvic region may be driven by tight muscles. Urinary hesitancy and erectile dysfunction are indicators of this muscle tension.
A quick tip for you may be to sit down while urinating. This will allow the muscles of your pelvic floor to stretch open and balloon downwards, thereby easing the flow of the urine stream.
For the erectile dysfunction, you can seek out pelvic floor physical therapy. This is treatable and so very common! Thanks for this message you wrote. Hopefully, it will encourage other men to find the help that they need.

@@TheRealBruceLouis This is such an excellent question! Sitting down while urinating is very helpful to men, especially if they have symptoms of pain in the pelvis, or the sensation of urinary hesitation, dribbling or incomplete voiding.
If you are sitting while urinating, try not to "push" the urine out. Instead, try to imagine that your perineum is ballooning out into the toilet bowl. Do some deep breathing in this position and you should feel your pelvic floor relax. The urine should just come out without pushing. The same goes for bowel movements.

I had an operation to remove a kidney stone and ever since I've had this. My urologist did an examination of my prostate and said it was fine but the muscles around it are really tight (hypo sensitive) is this my pelvic floor? Because I have pain in my abdomen, scrotum, tummy, bladder, penis, anus and painful erection and ejaculation. Not got a good urine floor as well It's HORRIBLE to live with! Oh, it's also hurts to sit down.

When I go out biking, a feel a pain between my testicles and anus I think its the pelvic floor area and when Im done biking I feel pain when Im peeing... is it a symptom of chronic male pelvic pain? and what is the cure or remedy?
Dear Naig, Yes, people with chronic male pain syndrome find riding a bike challenging, because the seat of the bike compresses the pelvic floor muscles. You can try looking for a specialty bike seat to take pressure off of this area (they sell them!). Alternatively, you could find another way to exercise or another form of transport.

Greetings, Humberto. Some men with pelvic floor muscle tension do report frequent urination. One suggestion we would make for them would be to sit down on the toilet seat while peeing. This serves the purpose of allowing the pelvic floor muscles to relax and thereby emptying the bladder more fully. We have seen this work in practice to decrease urinary frequency in men.
