1. when it become mobile (grade 2/3) due to bone loss 2. if the teeth is located within the cystic cavity 3. when it is not suitable for restoration, endodontically or prosthetically 4. other reasons - supernumerary / impacted teeth etc..
Extremely large mandibular cysts destruct most of the bone including lower border of the mandible in cyst region....so while attempting to separate cyst lining from underlying bone, due to that pressure it may result in fracture

not much problem with bone healing ...generally, apicectomy is done after enucleation when teeth are retained

diagnosis of true cyst or pseudocyst is done histopathologically...clinically there is no significant difference between the two...
