But it is now a world and he is real character now. He is not trapped. You can say he is fully transferred to this world. If he die, he will really die.

Ainz is shown here to be clueless of the reality that the user of the world item on shaltear is already dead killed by shalltear the moment before the effect takes in, his cluelessness here leads him to a conclusion that there is a player in this world at the time where he is now when the reality is that the user of that world item is an old woman who is a part of the black scripture from Slane Theocracy who is blessed with world item that was left by the players who built that nation.
