But what most assessors forget is the actual means for getting in and out (access / egress) example of this is 1min 48 sec! Part of your pre-task meeting for the entry should consider all aspects of the entry not just on the inside! It should include the a suitable and sufficient means for access and egress which in this case should be a scaffold access platform for the person entering to safely enter/exit and for emergency response crews to safety extract anyone.

Reenactments of confined space accidents and confined space rescues are used to teach the viewers of this video the importance of following OSHA regulations that require practices and procedures which protect workers from potential hazards during a confined space entry.
Here is what happened in this incident. The workers did not properly assess the confined space for any hazards in the atmosphere by using a air monitor. Because of that they did not realize that the atmosphere in the confined space was hazardous as what they didn’t know was that there was no oxygen in the confined space. After entering the confined space worker 1 was overcome due to the lack of oxygen. A 2nd worker saw that he was unconscious and desperately tried to rescue his fellow coworker by calling out for help. The 2nd person then instinctively went into the confined space in a attempt to rescue his coworker. Unfortunately he did not realize that there was no oxygen in the confined space and did not have any proper training or equipment such as breathing apparatus. The 3rd person who had heard the shouting came to the area and asked what was going on but as he said that the 2nd worker was himself overcome by the lack of oxygen. Upon seeing worker 1 unconscious and witnessing worker 2 being overcome worker 3 instinctively went into the confined space to rescue his coworkers not realizing the danger that was in the confined space. Unfortunately he like worker 2 did not have any proper training or equipment such as breathing apparatus. After entering the confined space worker 3 was also overcome by the lack of oxygen and went unconscious as well. Thankfully some other workers in the area were properly trained in confined space rescue and were able to remove the people from the confined space safely though unfortunately by then it was too late to save the 3 people who had been overcome by the lack of oxygen.
