Not on all (in europe at least), but if you have it, it is soo more easier. Had a US built car without FCC (Chrysler 2008.) Had some options, not 100% any of them, but it worked on my first try. So I might say, it all depends on Make/Market/Model and year.

If any of the steps fail do I have to worry about losing programming from the original key?
The vehicle would be in program mode and you continue to close the cycle and have one working key to start again.
