Try using DXVK on this game. Your GPU can do so much more, but DX9 is holding it back due to its single threaded nature. Your 0.1%, 1% lows and your minimum fps will go up alot. I tested my 7800X3D and RTX4080 on Fallout 4 and most of the stuttering goes away, meaning fewer 0.1% lows. Fallout 4 is DX11, which is still limiting but not as bad as DX9.
It's both impressive how good the game looks without mods and disappointing how more performance isn't gained without the thing your mentioning. But glad it exists

The current versions are:
DXVK 2.3 (no Async)
DXVK 2.0 (Async)
They both support DX9/10/11 titles.
I think DX12 to Vulkan exists, but I've never tried it.

Why does it look like the game is sped up to a commical 4x speed? Even the Enclave bot looks like its jumping around like a glitch, the wind sounds like you're running through a hurricane. Gotta be sped up considering these settings in game are locked at or below 60fps

Holy crap dude! While the framerate is perfectly acceptable, for a 16 year old title it sure does present one hell of a challenge for today's most powerful gaming graphics card. I mean, look at those damn percentile lows! Also, I would expect average FPS to be at least 2-3 time higher than what we saw here, regardless of resolution. What a beast, this game! I have no idea why Crysis is the one that's memed about its system requirements.
Maybe the PS5 could play this in 8K.
