1- The supporters are tired of what they think it’s a “corrupt” league where only the rich and powerful (Juventus) win. (You can’t really blame them since Juventus has a bad track record when it comes down to corruption and when their “victories” are always contested because of referees mistakes, usually in favor). Thus they see no point in sacrificing and going to the stadium.
2- the stadium itself is old, beat, battered and non functional. The restrooms haven’t worked in years, the parking situation is out of hand and illegal parking attendant charge up to €10 per car which adds to the ticket prices. You do not have assigned seats (trust me you don’t). Regardless, the San Paolo is still a gorgeous stadium tho.
3- animosity towards the main man. The chairman itself “Aurelio De Laurentis”. Lots of supporters don’t appreciate him (actually hate him).
4. In some cases, when there are back to back games it’s financially challenging for the supporters to come up with the money for 2 or 3 games in the span of 10-12 days. Thus, Neapolitans (inherently not wealthy) need to be parsimonious and choose wisely.
5- The terrible public transportation system from and to the stadium makes it hard and or outright impossible for thousands of neapolitans to make the trek. A lot of buses and trains stop running before most matches end. As a result passengers and supporters are stranded.
6- it’s just outright cold in :”December, January and February” and people don’t dare venture out and will watch the match in the comfort of their homes.
7- while the main goal is to enjoy a great soccer match, the stadium doesn’t offer much in terms of entertainment.
So, there you have it. I know it’s long but I hope it helps answer your question.

It depends case by case. Chievo and Sassuolo have a small fan base( especially Sassuolo has a stadium of 20k and town of 15k inhabitants while Chievo is a suburb of Verona, the majority of Verona's inhabitants support Hellas). Then there are a few club that rarely fill completely their stadium during "average matches": Bologna, Empoli, Lazio, Napoli( this year they have a lot of problems, last year was always full). Then, among the club that shares the same stadium (Genoa-Samp, Lazio-Roma) there is the problem to fill the "Curva" owned by the opponents, so for example in a Genoa match you will see the Sampdoria's Curva pretty empty, even if Genoa has a great fan base. Anyway there are several excellences in Serie A: Atalanta, Cagliari, Spal, Inter, Juventus, Milan, Udinese, Frosinone.. those teams have a high % of attendance. Of course you need to consider that 50% of the stadium are too old and offer poor services, ticket's prices are pretty high while italians still suffer economic diseases after 2008 crisis. Few clubs are moving forward anyway: Roma planned to built a new stadium, Bologna will renew the Dall'Ara, new stadium for Cagliari, Atalanta will finish to renew their stadium in a couple of years.
