What about the costs involved with having rural Americans get on planes to get treated at the Mecca for their condition? Many rural people have never been on an airplane and simply cannot afford the travel involved with traveling hundreds of miles for care, or they do not want to leave home. Rural people and their mindset about their lives is very different from that of a Harvard Professor.
