I've never managed it, but others recommended I summon gargantuans on top of elites when possible, as they are your strongest attack when using helltooth combined with the short man's finger. The long cooldown gave me enough time to forget them... The spirit barrage isn't boosted by helltooth, iirc. So gafla. Thank you for helping others.
