This has happened many times in Boston with the light-rail portals on Commonwealth, Beacon, and Huntington. Now it's L.A.'s turn. It will happen wherever street-level tracks enter a tunnel.
There's no way to fix stupid for incidents like these once and for all because it'll always happen, just not necessarily by the same person. Metro needs to grade-separate the E line so service would no longer be interrupted by surface traffic.
